摘要
目的 分析我国2009—2022年国家层面县级公立医院综合改革与发展政策文本的内容特点,为持续完善优化县级公立医院政策体系、推动其高质量发展提供科学参考。方法 采用内容分析法,基于构建的“基本政策工具—系统维度—发展阶段”三维分析框架,对国家层面县级公立医院改革发展的政策条目进行频数统计与交叉对比分析。结果 共纳入的95篇国家层面县级公立医院改革与发展相关政策文件,在3个发展阶段中,政策工具的开展总体上基本以环境型工具使用为主,供给型政策工具、需求型政策工具使用为辅的特点;系统维度工具的使用具有差异性,宏观维度居多;基本政策工具与系统维度工具之间的组合搭配较为合理,二者内部的子维度占比在各发展阶段维度中基本稳定。结论 政策制定者应结合战略需求与社会环境等发展背景,进一步统筹基本政策工具与系统维度工具运用的均衡性与合理性,合力推动新时代下县级公立医院改革拓展深化,向高质量发展目标迈进。
Objective To analyze the content characteristics of county-level public hospital reform and development policy at the national level from 2009 to 2022,and to provide scientific references for the subsequent improvement and optimization of the county-level public hospital policy system and promotion of its high-quality development. Methods Based on the three-dimensional analysis framework of“basic policy tools-system dimensions-deve lopment stages”,it conducted a frequency count and cross-comparison analysis of the national-level county-level public hospital reform and development policies using the content analysis method. Results Among the 95 national-level policy documents related to the reform and development of county-level public hospitals,the use of policy tools in the three development stages is generally characterized using environment-based tools, supplemented by supply-based and demand-based policy tools;the use of system-dimensional tools is different,with the macro-dimension predominating;the combination of basic policy tools and system-dimensional tools is reasonable. The combination of basic policy tools and systemic tools is reasonable,and the proportion of sub-dimensions within the two is basically stable in each development stage dimension. Conclusion Policymakers should further coordinate the balance and rationality of the use of basic policy tools and systemic tools in the context of strategic needs and social environment to promote the reform of county-level public hospitals in the new era and move toward the goal of high-quality development.
