p16蛋白与HPV mRNA检测在口咽部鳞状细胞癌诊断中的意义 被引量:1

摘要 目的探讨p16蛋白与HPV mRNA检测在口咽部鳞状细胞癌(oropharyngeal squamous cell carcinoma,OPSCC)诊断中的意义。方法收集2019年11月~2021年10月首都医科大学附属北京同仁医院诊治的105例OPSCC,采用免疫组化EnVision两步法检测p16、Ki-67、p53蛋白表达,并检测其中61例HPV16/18 mRNA的表达,分析其与临床病理特征的关系。结果105例OPSCC中p16蛋白阳性58例(55.24%),阴性47例(44.76%);p16蛋白阳性患者比阴性患者的吸烟、饮酒史比例低(26/58 vs 37/47,16/58 vs 31/47),以扁桃体为常见发病部位(51/58 vs 18/47),易出现淋巴结转移(43/58 vs 23/47),Ki-67增殖指数较高(73.10%vs 47.45%),p53多为野生型(57/58 vs 16/47),形态学特征以非角化型鳞状细胞癌为主(29/58 vs 15/47),差异均有统计学意义(P<0.05)。61例HPV mRNA检测结果与p16蛋白表达完全一致(Kappa=1)。43例p16蛋白阳性OPSCC患者获得随访,3例复发/转移,1例肺转移后死亡;39例p16蛋白阴性OPSCC患者获得随访,8例复发/转移,7例死亡,其中2例为复发后死亡。结论HPV感染导致OPSCC发病率较高,以扁桃体为常见发病部位,p16蛋白表达与HPV mRNA表达具有较好的一致性,临床工作中可使用p16免疫组化检测代替HPV分子检测,进行HPV的感染筛查及辅助诊断。 Purpose To investigate the significance of p16 protein and HPV mRNA detection in the diagnosis of oropharyngeal squamous cell carcinoma(OPSCC).Methods The clinical and pathological data of 105 cases of OPSCC were collected from the Department of Pathology,Beijing Tongren Hospital,Capital Medical University from November 2019 to October 2021.The expression of p16,Ki-67 and p53 protein was detected by immunohistochemistry and the mRNA expression of HPV16/18 in 61 cases was detected.Statistically analysis was used to access the association between the detection results and clinicopathological characteristics.Results Among the 105 OPSCC cases,58 cases were in the p16 protein-positive OPSCC group,and 47 cases were in the p16 protein-negative OPSCC group.The former accounted for a higher proportion than the latter(55.24%vs 44.76%).Compared with the p16 protein-negative OPSCC group,the proportion of smoking and drinking history in the p16 protein-positive OPSCC group was lower(26/58 vs 37/47,16/58 vs 31/47),tonsil was the common site of disease(51/58 vs 18/47),and lymph node metastasis was more likely to occur(43/58 vs 23/47),Ki-67 showed a high proliferation index(73.10%vs 47.45%),p53 was mostly wild-type(57/58 vs 16/47),and morphological characteristics were mainly non-keratinizing squamous cell carcinoma(29/58 vs 15/47).The difference between two groups showed statistical significance(P<0.05).The detection results of HPV mRNA in 61 cases were completely consistent with the results of p16 protein expression(Kappa=1).Follow-up information was obtained for 43 cases of p16 protein-positive OPSCC:3 cases of recurrence/metastasis,and 1 case of death after lung metastasis.39 cases of p16 protein-negative OPSCC obtained follow-up information:8 cases of recurrence/metastasis,and 7 cases of death,of which 2 cases died after recurrence.Conclusion The incidence of OPSCC caused by HPV infection is relatively high,and tonsil is the common site of disease.p16 protein expression is in good agreement with HPV mRNA expression.In clini
