HPV E6/E7 mRNA检测联合TCT在子宫颈病变早期筛查中的应用效果 被引量:4

摘要 目的研究人乳头瘤病毒(HPV)E6/E7 mRNA检测联合薄层液基细胞学检查(TCT)用于早期筛查子宫颈病变的临床价值。方法回顾性收集2019年1月至2021年3月于本院妇科门诊诊治的262例可疑子宫颈病变患者的临床资料,所有患者均接受HPV E6/E7 mRNA检测、TCT、阴道镜及病理组织学检查。以阴道镜及病理组织学检查结果为金标准,比较HPV E6/E7 mRNA检测、TCT单独及联合检测对子宫颈病变的诊断效能。结果随着子宫颈病变程度的加重,HPV E6/E7 mRNA检测、TCT单独及联合检测的阳性检出率基本呈逐渐升高趋势。联合检测对子宫颈病变的诊断灵敏度、特异度高于HPV E6/E7 mRNA检测、TCT单独检测(P<0.05);HPV E6/E7 mRNA检测对子宫颈病变的诊断特异度高于TCT(P<0.05)。结论HPV E6/E7 mRNA检测联合TCT能够明显提高子宫颈病变早期筛查的灵敏度和特异度,其在子宫颈病变筛查中具有较高的临床应用价值。 Objective To investigate the clinical value of human papillomavirus(HPV)E6/E7 mRNA detection combined with thinprep cytologic test(TCT)for early screening of cervical lesions.Methods The clinical data of 262patients with suspected cervical lesions treated in the gynecology clinic of our hospital from January 2019 to March 2021were retrospectively collected.All patients received HPV E6/E7 mRNA detection,TCT,colposcopy and histopathology examination.Taking colposcopy and histopathology examination as the gold standard,the diagnostic efficiency of HPV E6/E7mRNA detection,TCT single and combined detection for cervical lesions were compared.Results With the aggravation of cervical lesions,the positive detection rate of HPV E6/E7 mRNA detection,TCT single and combined detection basically increased gradually.The diagnostic sensitivity and specificity of combined detection for cervical lesions were higher than those of HPV E6/E7 mRNA detection and TCT single detection(P<0.05);the diagnostic specificity of HPV E6/E7 mRNA detection for cervical lesions was higher than that of TCT(P<0.05).Conclusion HPV E6/E7 mRNA detection combined with TCT can significantly improve the sensitivity and specificity of early screening of cervical lesions,which has high clinical application value in cervical lesions screening.
