摘要
卡铂是一种广泛用于多种恶性肿瘤化疗的铂类抗癌药,以卡铂为基础的联合化疗已成为治疗妇科恶性肿瘤化疗的一线方案。卡铂有发生过敏反应的风险,且随着疗程增加而增大。如何安全使用卡铂,预测其过敏反应以及卡铂过敏患者的后续治疗是临床亟待解决的问题。该文阐述了卡铂过敏反应的临床分级、诊断标准、作用机制、影响因素以及卡铂皮试在管理患者中的应用,提出了安全有效的单一溶液脱敏方案以及卡铂发生过敏反应的处理流程,给卡铂在临床的安全使用提供了重要的参考依据。
Carboplatin is a platinum anticancer drug widely used for chemotherapy of various malignant tumors. Carboplatin-based combined chemotherapy has become the first-line chemotherapy for gynecological malignant tumors. Carboplatin has a risk of hypersensitivity reactions, and the risk of hypersensitivity reactions increases with the course of treatment. How to use safely, predicting its hypersensitivity reactions and the subsequent treatment are urgent problems to be solved clinically. This paper expounded the scientific clinical classification, diagnostic criteria, mechanism of action and influencing factors of carboplatin-induced anaphylactic reaction, as well as the application of carboplatin skin test in the management of patients, and put forward a safe and effective single solution desensitization scheme, and the treatment process of carboplatin hypersensitivity reactions, so as to provide important reference for the safe use of carboplatin in clinic.
