摘要
目的:系统评价2型糖尿病病人发生视网膜病变的影响因素。方法:计算机检索PubMed、Web of Science、the Cochrane Library、EBSCO、Springer Link、ScienceDirect、Willey、中国知网(CNKI)、维普数据库(VIP)、万方数据库关于2型糖尿病病人并发视网膜病变影响因素的研究,检索时限为建库至2021年12月15日,采用RevMan 5.3软件进行Meta分析。结果:共纳入20项研究,涉及14812例病人,共20个影响因素。Meta分析结果显示,2型糖尿病病人发生视网膜病变的主要影响因素为年龄[SMD=0.59,95%CI(0.20,0.97),P=0.003]、糖尿病病程[SMD=0.97,95%CI(0.60,1.33),P<0.00001]、收缩压[SMD=0.17,95%CI(0.07,0.27),P=0.0007]、糖化血红蛋白[SMD=0.46,95%CI(0.29,0.64),P<0.00001]、三酰甘油[SMD=0.21,95%CI(0.02,0.40),P=0.03]、餐后2 h血糖[SMD=0.53,95%CI(0.04,1.03),P=0.03]、血肌酐[SMD=0.43,95%CI(0.20,0.66),P=0.0002]。结论:现有证据表明,年龄、糖尿病病程、收缩压、糖化血红蛋白、三酰甘油、餐后2 h血糖、血肌酐是2型糖尿病病人发生视网膜病变的影响因素,临床医护人员应注意识别2型糖尿病视网膜病变的高危人群,及时采取针对性的预防措施以降低该人群发生视网膜病变的风险。
Objective:To systematically evaluate the influencing factors of retinopathy in type 2 diabetic patients.Methods:The research on the influencing factors of type 2 diabetes patients complicated with retinopathy was retrieved from PubMed,Web of Science,the Cochrane Library,EBSCO,Springer Link,Science Direct,Willey,CNKI,VIP,WanFang Database.The retrieval time limit was from the establishment of the database to December 15,2021.RevMan 5.3 software was used for Meta-analysis.Results:A total of 20 articles were included,involving 14812 patients and 20 influencing factors.Meta-analysis results showed that the main influencing factors of retinopathy in type 2 diabetes patients were age[SMD=0.59,95%CI(0.20,0.97),P=0.003],duration of diabetes[SMD=0.97,95%CI(0.60,1.33),P<0.00001],systolic blood pressure[SMD=0.17,95%CI(0.07,0.27),P=0.0007],glycosylated hemoglobin[SMD=0.46,95%CI(0.29,0.64),P<0.00001],triacylglycerol[SMD=0.21,95%CI(0.02,0.40),P=0.03],2 h postprandial blood glucose[SMD=0.53,95%CI(0.04,1.03),P=0.03]and serum creatinine[SMD=0.43,95%CI(0.20,0.66),P=0.0002].Conclusion:The existing evidence shows that age,duration of diabetes,systolic blood pressure,glycosylated hemoglobin,triacylglycerol,postprandial 2 h blood glucose,and serum creatinine are the influencing factors of retinopathy in type 2 diabetes patients.Clinical medical staff should pay attention to identifying high-risk groups of type 2 diabetes retinopathy,and timely take targeted preventive measures to reduce the risk of retinopathy in this group.
