摘要
目的:分析尘肺患者64排螺旋CT肺纤维化评分与肺功能的关系。方法:选取南京市职业病防治院2021年7月—2022年8月放射科接诊的67例尘肺患者,将其设为观察组;另选取同期于本院体检的70名健康体检者,将其设为健康组。所有纳入对象均进行64排螺旋CT、肺功能检查,比较观察组、健康组肺纤维化评分、肺功能指标,比较1期、2期、3期尘肺组肺纤维化评分、肺功能指标,对肺纤维化评分与肺功能的相关性进行Spearman相关性分析。结果:观察组肺纤维化评分显著高于健康组,FEV_(1)、FVC、FEV_(1)/FVC值均显著低于健康组(P<0.01)。3期组肺纤维化评分高于2期组、1期组,3期组FEV_(1)、FVC、FEV_(1)/FVC均低于2期组、1期组,差异均具有统计学意义(P<0.05)。肺纤维化评分与FEV_(1)、FVC、FEV_(1)/FVC均呈负相关性(r值=-0.428、-0.457、-0.469,P<0.05)。结论:尘肺患者肺纤维化评分与肺功能存在一定的相关性,临床可通过64排螺旋CT检查,准确评估患者肺纤维化程度,从而指导临床展开针对性治疗。
Objective To analyze the relationship between 64-slice spiral CT pulmonary fibrosis score and pulmonary function in patients with pneumoconiosis.Methods A total of 67 patients with pneumoconiosis who were admitted to the radiology department of Nanjing Occupational Disease Prevention Hospital from July 2021 to August 2022 were selected and set as the observation group;In addition,70 healthy medical examiners who were examined at our hospital during the same period were selected and set as the healthy group.All included participants underwent 64-slice spiral CT and pulmonary function tests,and the pulmonary fibrosis scores and pulmonary function indexes of the observation group and the healthy group were compared,and the pulmonary fibrosis scores and pulmonary function indexes of the stage 1,2 and 3 pneumoconiosis groups were compared.Spearman correlation analysis was performed to analyze the correlation between the pulmonary fibrosis scores and pulmonary function Results The pulmonary fibrosis score in the observation group was significantly higher than that in the healthy group,and the FEV_(1),FVC,and FEV_(1)/FVC were significantly lower than those in the healthy group(P <0.01).The pulmonary fibrosis score in the stage 3 group was higher than that in the stage 2 group and the stage 1 group,and the FEV_(1),FVC,and FEV_(1)/FVC in the stage 3 group were lower than those in the stage 2 group and the stage 1 group,all differences were statistically significant(P <0.05).Pulmonary fibrosis score was negatively correlated with FEV_(1),FVC,and FEV_(1)/FVC(r=-0.428,-0.457,-0.469,P <0.05).Conclusion There is a certain correlation between pulmonary fibrosis score and pulmonary function in patients with pneumoconiosis.Clinically,64-slice spiral CT examination can accurately assess the degree of pulmonary fibrosis in patients,so as to guide clinical targeted treatment.
