摘要
目的:研究《名医类案》中“固本培元”法治疗咳嗽的组方规律和诊疗特色。方法:以咳嗽、咳痰、气喘、发热、鼻塞等符合咳嗽诊断标准的主症作为筛选依据,收集《名医类案》中记载的所有相关医案处方记录,使用Excel 2019、SPSS Modeler 18.0软件对纳入中药的频数、性味、归经、功效、关联度进行统计并分析。结果:纳入符合研究方剂145首,涉及中药142味,中药累计频数为1294。其中,使用频数≥18的中药共20种,频数最高的药物依次是人参(79%)、白术(70%)、炙甘草(59%)、陈皮(54%)、茯苓(49%)等;药物“四气”使用频数由大到小依次为温、微温、平、微寒、寒、热、凉;“五味”依次为甘、辛、苦、酸、咸;归经依次为脾、肺、胃、心、肾、肝等;补虚药、解表药、化痰药、清热药在功效分类中最为常见;关联规则显示关联强度最高的药对以补虚药为主;聚类分析结果共分为六类,主成分分析结果共提取公因子14项。结论:《名医类案》治疗咳嗽病以培补脾肺肾为本,以温补为主,兼行气养血、寒温并用、培土生金;其组方规律与固本培元思想之培补元气、温补脾肾的核心思想相符,两者交叉互融。
Objective:To study the rules of prescription and the characteristics of diagnosis and treatment of cough with the method of"strengthening the foundation and strengthening the vitality"in the Mingyi Leian.Methods:Using the symptoms of cough,cough and sputum,asthma,fever and nasal congestion as the basis for selection,all relevant prescription records of medical cases recorded in the Mingyi Leian were collected,and a database was established.The frequency,flavour,meridian tropism,efficacy and correlation of traditional Chinese medicine were counted and analysed using Excel 2019 and SPSS Modeler18.0 software.Results:A total of 145 prescriptions were included in the study,involving 142 Chinese medicines,and the cumulative frequency of Chinese medicines was 1294.Among them,a total of 20 kinds of traditional Chinese medicines with a frequency of≥18 were used.The most frequently used drugs were Renshen(Ginseng Radix)(79%),Baizhu(70%),Zhigancao(Glycyrrhizae Radix cum Liquido Fricta)(59%),Chenpi(Citri Reticulatae Pericarpium)(54%),Fuling(Poria)(49%),etc.The frequency of drug'four Qi'from large to small was warm,slightly warm,flat,slightly cold,cold,hot and cool.The'five flavors'are sweet,pungent,bitter,sour and salty in descending order.The meridians are spleen,lung,stomach,heart,kidney,liver,etc.The drugs for tonifying deficiency,relieving superficies,resolving phlegm and clearing heat are the most common in the efficacy classification.The association rule shows that the drug pair with the highest association strength is mainly deficiency medicine.The results of cluster analysis are divided into six categories,and 14 common factors are extracted from the results of principal component analysis.Conclusion:The treatment of cough in Mingyi Leian is based on nourishing the spleen,lung and kidney,mainly on warming,and also on promoting Qi and blood,cold and warm,and cultivating soil to generate gold.Its formulation law is consistent with the core idea of strengthening the vital energy and warming the spleen and kidney of th
