摘要
目的观察腹部穴位定向透药法和生活方式干预法治疗老年单纯性肥胖痰湿质患者的疗效及安全性。方法选择2019年1月—2020年6月在上海市杨浦区中医医院就诊的老年单纯性肥胖痰湿质患者80例,随机分为对照组和药透组,每组40例。对照组予生活方式干预法治疗,药透组予腹部穴位透药联合生活方式干预法治疗,2组均治疗4周。比较2组治疗前后体质指数(BMI)、中医体质评分、血压、腰围,检测2组治疗前后血脂、瘦素、脂联素、白细胞介素-6(IL-6)、胰岛素抵抗指数(HOMA-IR)、肝肾功能,评估2组治疗4周后疗效及安全性。结果药透组和对照组各33例完成研究。治疗4周后,药透组中医体质评分、腰围及三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、瘦素、HOMA-IR改善情况均明显优于对照组(P均<0.05),2组BMI、血压及总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、脂联素、IL-6水平比较差异均无统计学意义(P均>0.05)。治疗过程中没有患者出现肝肾损伤及胃肠道不适症状。结论腹部穴位药透联合生活干预可有效改善老年单纯性肥胖痰湿质患者的糖脂代谢,且安全。
Objective It is to observe the efficacy and safety of directional drug penetration at abdominal acupoint on abdominal acupoints and lifestyle intervention in the treatment of elderly patients with simple obesity and phlegm-dampness constitution.Methods A total of 80 elderly patients with simple obesity and phlegm-dampness constitution treated in Yangpu District Hospital of Traditional Chinese Medicine from January 2019 to June 2020 were selected and randomly divided into a control group and a drug penetration group,with 40 cases in each group.The control group was treated with lifestyle intervention,and the drug penetration group was treated with drug penetration at abdominal acupoint combined with lifestyle intervention.Both groups were treated for 4 weeks.The body mass index(BMI),scores of traditional Chinese medical constitution,blood pressure and waist circumference before and after treatment were compared in the two groups,the levels of blood lipids,leptin,adiponectin,interleukin-6(IL-6),and insulin resistance index(HOMA-IR),liver and kidney function were detected before and after treatment,the efficacy and safety of the 2 groups after 4 weeks of treatment were evaluated.Results Both 33 cases completed the study in the drug penetration group and the control group.After 4 weeks of treatment,the improvement of scores of traditional Chinese medical constitution,waist circumference,triacylglycerol(TG),high-density lipoprotein cholesterol(HDL-C),leptin,and HOMA-IR in the drug penetration group were significantly better than those in the control group(all P<0.05),there was no significant difference in BMI,blood pressure,and the levels of total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C),adiponectin,and IL-6 between the two groups(all P>0.05).During the treatment,no patients experienced liver and kidney damage and gastrointestinal discomfort.Conclusion Drug penetration at abdominal acupoint combined with life intervention can effectively and safely improve the glucose and lipid metabolism of elderly
