慢性鼻窦炎伴鼻息肉患者继发嗅觉障碍预测模型构建研究 被引量:2

摘要 目的探讨慢性鼻窦炎伴鼻息肉(chronic rhinosinusitis with nasal polyps,CRSwNP)患者继发嗅觉障碍影响因素并构建预测模型。方法回顾性分析河南省中医院2016年1月~2020年6月收治行鼻内镜手术治疗CRSwNP患者共176例临床资料,根据是否继发嗅觉障碍分组,比较两组一般资料、改良鼻窦CT双侧嗅区评分及实验室检查指标,采用Logistic回归模型分析CRSwNP患者继发嗅觉障碍独立影响因素,描绘受试者工作特征(receiver operating characteristic,ROC)曲线评价预测模型临床效能。结果两组左侧/右侧/双侧改良鼻窦CT双侧嗅区评分总分、双侧总吸气、组织嗜酸性粒细胞计数、血嗜酸性粒细胞比例及血尿素氮(BUN)水平比较差异有统计学意义(P<0.05);多因素Logistic回归模型分析结果显示,改良鼻窦CT双侧嗅区评分总分和BUN水平是CRSwNP患者继发嗅觉障碍独立影响因素(P<0.05);描绘ROC曲线计算预测模型AUC=0.88(95%CI:0.81~0.97),敏感度和特异度分别为87.26%和91.60%。结论CRSwNP患者继发嗅觉障碍发生与改良鼻窦CT双侧嗅区评分总分和BUN水平密切相关;同时基于可能影响因素构建预测模型具有良好临床效能。 OBJECTIVE To investigate the influencing factors of secondary olfactory dysfunction in patients with CRSwNP and the clinical efficacy of prediction model.METHODS Clinical data of 176 patients with CRSwNP treated by endoscopic sinus surgery were retrospectively chosen in the period from January 2016 to June 2020.All patients were grouped according to whether secondary olfactory dysfunction or not.The general data,modified bilateral nasal CT olfactory area score and laboratory examination indexes of the 2 groups were compared respectively.Logistic regression model was used to analyze the independent influencing factors of secondary olfactory dysfunction in patients with CRSwNP and ROC curve was drawn to evaluate the clinical efficacy of the prediction model.RESULTS There were significant differences in the total score of left/right/both bilateral olfactory area,total inspirations,eosinophil count,blood eosinophil ratio and BUN level between the 2 groups(P<0.05).Multivariate logistic regression analysis showed that the total score of bilateral olfactory area score and BUN level were independent factors of secondary olfactory dysfunction in patients with CRSwNP(P<0.05).To Depict ROC curve and calculate prediction model AUC=0.88(95%CI:0.81-0.97),sensitivity and specificity were 87.26%and 91.60%respectively.CONCLUSION The occurrence of secondary olfactory dysfunction in CRSwNP patients is closely related to the total score of bilateral olfactory area score of modified sinus CT and BUN level,and the prediction model based on the possible influencing factors has satisfactory clinical efficacy.
