摘要
目的观察系统性红斑狼疮(SLE)患者血清中高迁移率族蛋白B1(HMGB1)、白细胞介素(IL)-6及IL-9的水平及其与临床指标的相关性。方法选取2019年2月1日至10月31日于中国医科大学附属盛京医院风湿免疫科初治的20例SLE患者,选取同期健康体检志愿者20例为健康对照组,酶联免疫吸附试验(ELISA)检测所有受试者血清中HMGB1、IL-6及IL-9的表达水平,并分析三者与SLE患者其他主要临床指标的相关性。结果SLE患者血清HMGB1、IL-6及IL-9的表达水平均显著高于健康对照组,差异有统计学意义(P<0.05)。SLE患者血清HMGB1的表达水平与系统性红斑狼疮疾病活动指数(SLEDAI)评分、24 h尿蛋白定量、尿微量白蛋白、尿转铁蛋白、抗核抗体滴度、免疫球蛋白G水平、红细胞沉降率呈正相关关系(r=0.681,0.645,0.687,0.729,0.635,0.697,0.655;P<0.05),与补体C3、补体C4、维生素D3水平、白细胞计数、血红蛋白呈负相关关系(r=-0.558,-0.730,-0.610,-0.537,-0.665;P<0.05);IL-6水平与SLEDAI评分及红细胞沉降率呈正相关(r=0.502,0.507;P<0.05);IL-9与病史时间呈正相关(r=0.549,P<0.05);HMGB1、IL-6及IL-9之间无显著相关性(P>0.05)。结论细胞因子HMGB1及IL-6可一定程度上反映疾病活动度,HMGB1升高可能参与SLE肾脏受累及血液系统受累致病过程,未发现IL-9与疾病活动相关。
Objective To investigate the serum levels of high mobility group box(HMGB)1,interleukin(IL)-6 and IL-9in patients with systemic lupus erythematosus(SLE)and their association with clinical parameters.Methods Enzymelinked immunosorbent assay was used to check the levels of HMGB1,IL-6 and IL-9 in the serum of 20 SLE patients receiving initial treatment and 20 healthy controls in Shenjing Hospital from Feb.1,2019 to Oct.31,2019.The correlation between these 3 factors and other major clinical indicators in SLE patients was analyzed.Results The serum levels of HMGB1,IL-6 and IL-9 in patients with SLE were significantly higher than those in healthy volunteers(P<0.05).In SLE patients,the serum level of HMGB1 was positively correlated with SLEDAI score,24 h urine protein quantitation,urinary microalbumin,urinary transferrin,antinuclear antibody titer,immunoglobulin G level and erythrocyte sedimentation rate in patients with SLE,respectively(r=0.681,0.645,0.687,0.729,0.635,0.697,0.655;P<0.05);the level of HMGB1 was negatively correlated with the levels of complement C3,complement C4,vitamin D3levels,white blood cell count and hemoglobin in patients with SLE(r=-0.558,-0.730,-0.610,-0.537,-0.665;P<0.05);the level of IL-6 was positively correlated with SLEDAI score and ESR(r=0.502,0.507;P<0.05);the level of IL-9 was positively correlated with the duration of disease(r=0.549,P<0.05).There was no significant correlation among HMGB1,IL-6 and IL-9(P>0.05).Conclusion Cytokine HMGB1 and IL-6 can reflect disease activity to a certain extent.HMGB1 increaes can indicate renal involvement and involvement of blood system,while IL-9 has not been found to be associated with disease activity.
