摘要
目的 探讨便携式踝臂指数(ABI)测量仪在下肢动脉疾病(LEAD)中的临床应用。方法 选取2021年3月至2022年3月解放军总医院第一医学中心收治的120例LEAD患者为研究对象,每例患者分别采用多普勒超声-ABI(Dop-ABI)、便携式智能化-ABI(Port-ABI)设备进行检查。比较两种方法的测量时间,分析不同Fontaine分期、不同性别及是否合并糖尿病(DM)的LEAD患者中两种测量方法的组内相关系数(ICC)。结果 Port-ABI测量时间短于Dop-ABI,差异有统计学意义(P<0.05)。Fontaine分期Ⅰ~Ⅳ期LEAD患者两种检测方法的ICC分别为0.908、0.817、0.466、0.391;男性、女性LEAD患者两种检测方法的ICC分别为0.874、0.931;DM和非DM的LEAD患者两种检测方法的ICC分别为0.587、0.922。结论 Port-ABI是一种新型ABI检测设备,在无症状或轻中度临床症状的LEAD患者中与Dop-ABI具有良好的一致性,但在重度下肢缺血中一致性较差,因此更适用于LEAD的早期诊断和筛查。
Objective To investigate the clinical application of ankle-brachial index(ABI) in lower extremity artery disease(LEAD). Method A total of 120 LEAD patients admitted to the First Medical Center, Chinese People’s Liberation Army General Hospital from March 2021 to March 2022 were selected as the research subjects. Patients were examined with Doppler ultrasound-ABI(Dop-ABI) and portable intelligent-ABI(Port-ABI) equipment. The measurement times of the two methods were compared. The intraclass correlation coefficient(ICC) of the two measurement methods in different Fontaine stages, different genders, and whether complicated with diabetes mellitus(DM) were also analyzed.Result The measurement time of Port-ABI was shorter than that of Dop-ABI, the difference was statistically significant(P<0.05). The ICC values of the two detection methods were 0.908, 0.817, 0.466, and 0.391, respectively in Fontaine stage I-IV of LEAD patients. The ICC values of the two detection methods for male and female of LEAD patients were 0.874 and 0.931, respectively. The ICC values of the two detection methods for DM and non-DM of LEAD patients were 0.587 and 0.922, respectively. Conclusion Port-ABI is a novel ABI detection device, has good consistency with DopABI in asymptomatic or mild-to-moderate LEAD patients. However, its consistency in severe lower extremity ischemia is poor. Therefore, it is more suitable for early diagnosis and screening of LEAD.
