摘要
目的评估康复训练+心理干预对改善老年脑卒中患者吞咽功能障碍(DAS)负性情绪的效果。方法纳入2020年8月~2021年8月的74例老年脑卒中后吞咽功能障碍(DAS)患者,遵循随机数字表法将病例分配至对照组(纳入37例,提供康复训练)观察组(纳入37例,提供康复训练+心理干预),比较组间吞咽生存质量问卷(SWAL-QOL)负性情绪评分、中文版脑卒中康复自我效能量表(SSEQ)、美国国立卫生研究院脑卒中量表(NIHSS)、康复依从性和吞咽功能改善情况。结果(1)治疗8周后,观察组SWAL-QOL评分、负性情绪评分、SSEQ评分、NIHSS评分较治疗前显著改善,P<0.05;(2)在康复依从率方面,观察组(97.29%)高于对照组(83.78%),P<0.05;(3)在吞咽功能改善有效率方面,观察组(97.29%)高于对照组(78.38%),P<0.05。结论对老年DAS患者实施康复训练+心理干预,能够提高康复依从性及自我效能感,尽快恢复吞咽功能,改善神经功能及生存质量,减轻负性情绪。
Objective To evaluate the effect of rehabilitation training and psychological intervention on swallowing dysfunction and negative emotion in elderly stroke patients. Methods 74 elderly patients with post-stroke swallowing dysfunction(DAS)from August 2020 to August 2021 were included. The cases were randomly assigned to the control group(37 cases were included and provided with rehabilitation training),the observation group(37 cases were included and provided with rehabilitation training + psychological intervention). The swallowing quality of life questionnaire(SWAL-QOL),negative emotion score,Chinese version of stroke rehabilitation self-efficacy scale(SSEQ)National Institutes of Health Stroke Scale(NIHSS),rehabilitation compliance and improvement of swallowing function were compared between two groups. Results (1)After 8 weeks of treatment,the SWAL-QOL score,negative emotion score,SSEQ score and NIHSS score of the observation group were better,P<0.05.(2)In terms of rehabilitation compliance rate,the observation group(97.29%)was higher than the control group(83.78%),P<0.05.(3)The effective rate of swallowing function improvement in the observation group(97.29%)was higher than that in the control group(78.38%),P<0.05. Conclusion Rehabilitation training and psychological intervention can improve their rehabilitation compliance and self-efficacy,recover their swallowing function as soon as possible,improve their neurological function and quality of life,reduce negative emotions in elderly DAS patients.
