摘要
目的动态分析2016-2020年天津市保健门诊儿童血铅水平的变化,为儿童铅暴露的防治工作提供科学依据。方法收集2016年1月-2020年12月在天津市妇女儿童保健中心儿童保健门诊(包括行为发育门诊)就诊儿童进行血铅检测的相关资料,比较5 a儿童血铅水平变化情况及性别年龄间的差异。结果共检测59203人次,其中男童34230人次,占57.82%,女童24973人次,占42.18%,血铅中位数(M)和四分位间距(P25,P75)为57.86(44.23,69.84)μg/L。2016-2020年,血铅中位数分别为69.40、64.73、53.71、39.53、35.66μg/L,逐年下降且各年比较,差异有统计学意义(χ^(2)=31093.101,P<0.01);高铅血症检出率分别为7.00%、2.88%、1.14%、0.88%、0.84%,男童血铅中位数为58.68μg/L,高于女童血铅中位数56.79μg/L,差异有统计学意义(Z=13.529,P<0.01)。各年度儿童血铅水平随年龄增长有增高趋势,不同年龄组血铅水平比较,差异有统计学意义(P<0.01)。发育行为门诊儿童血铅水平为61.49μg/L,明显高于其他保健门诊儿童血铅水平55.46μg/L,差异有统计学意义(Z=32.795,P<0.01)。结论2016-2020年本市保健门诊儿童血铅中位数呈逐年下降趋势,儿童血铅水平存在性别、年龄差异,与儿童行为问题有一定相关性。
Objective To explore the trends of children’s blood lead levels in child care clinics in Tianjin from 2016 to 2020,and to provide scientific basis for prevention and treatment of children’s lead exposure.Methods All data of children’s blood lead levels collected from child care clinics of Tianjin women and children health center from January 2016 to December 2020 were analyzed,and the changes of children’s blood lead levels and the differences between gender and age in the past five years were compared.Results During the five years,59203 children were detected,including 34230(57.82%)boys and 24973(42.18%)girls.The median(M)and inter-quartile(P,P)of blood lead were 57.86(44.23,69.84)μg/L.From 2016 to 2020,the median(M)of blood lead were 69.40,64.73,53.71,39.53,and 35.66μg/L.The levels of blood lead decreased year by year and the difference was significant(χ^(2)=31093.101 P<0.01).In recent five years,the detection rates of high blood lead were 7.0%,2.88%,1.14%,0.88%and 0.84%.Before puberty,blood lead levels increased with age.The blood lead levels of boys(58.68μg/L)were higher than that of girls(56.79μg/L),and the difference is significant(Z=13.529,P<0.01).The blood lead levels of children increased with age,and the difference was statistically significant(P<0.01).Blood lead level of children in developmental behavior clinic(61.49μg/L)was significantly higher than that in health care clinic(55.46μg/L),and the difference was statistically significant(Z=32.795,P<0.01).Conclusion From 2016 to 2020,the median blood lead level of outpatient children in our city is decreasing year by year,and there are gender and age differences in children blood lead level,and it is related to the problem of children,sbehavior.
