摘要
目的:分析近5年来国内外基于fMRI的针刺脑效应研究现状。方法:运用文献计量学研究方法分析近5年公开发表在学术期刊上的fMRI针刺脑效应研究文献。结果:纳入的文献多得到省级以上科研基金资助。研究内容以病理状态下组合穴位针刺脑效应变化和影响因素为主,受试者以神经系统疾病和慢性疼痛性疾病居多,针刺操作以手针、电针为主。结论:当前针刺fMRI研究已由探索基本原理进入探索病理状态下针刺脑效应变化,为临床疗效提供证据为主,但多使用单一分析方法分析,大部分研究样本量较小。后续研究应加大样本量,规范试验设计,注重临床实际情况下的多穴位组合,行多模态fMRI检测,并与临床多种检测指标进行相关分析,增加随访研究,为针刺疗效评价及其作用机制研究提供更实用、更有价值的科学依据。
Objective:This paper aims to analyze the research status of brain effects of acupuncture based on resting-state fMRI(rs-fMRI)at home and abroad in the past 5 years.Methods:The literature published in academic journals in the past five years about the acupuncture effects of task and rs-fMRI of the brain was analyzed by bibliometrics.Results:Most of the included studies were funded by scientific research funds at the state or provincial level.The focuses were the changes and influencing factors of brain effects of acupuncture at a single point under pathological conditions.The subjects were mostly nervous system diseases and chronic pain disorders.The main types of acupuncture were manual acupuncture and electroacupuncture.Conclusions:The current acupuncture fMRI studies have shifted from exploring the basic acupuncture principles to investigating the changes in brain effects by acupuncture in pathological conditions to provide imaging evidence for clinical efficacy.Most of the studies are used a single analysis method in a small sample size.Future studies should increase the sample size,standardize the experimental design,pay attention to the combination of multiple acupoints under the actual clinical conditions,conduct multi-modal fMRI detection,conduct correlation analysis with various clinical detection indicators,and increase follow-up studies,so as to provide more practical and valuable scientific basis for acupuncture efficacy and its mechanism of action.
