新生儿肺炎血清肺表面活性物质及急性时相反应蛋白的表达变化观察

摘要 目的:探究及观察新生儿肺炎血清肺表面活性物质及急性时相反应蛋白的表达变化情况。方法:选取2019年3月-2022年2月厦门大学附属中山医院收治的150例肺炎新生儿,根据患儿的肺炎严重程度分为三组,重度新生儿肺炎[肺超声评分(LUS)≥18分]为A组,中度新生儿肺炎(LUS 11~17分)为B组,轻度新生儿肺炎(LUS≤10分)为C组,各50例;另选取同时期的50例健康新生儿为D组。检测及比较四组血清肺表面活性物质[肺表面活性物质相关蛋白-A(SPA)、肺表面活性物质相关蛋白-B(SP-B)及肺表面活性物质相关蛋白-C(SP-C)]及急性时相反应蛋白[C反应蛋白(CRP)、α_(1)-酸性糖蛋白(α_(1)-AG)及铜蓝蛋白(CER)]表达水平,分析新生儿肺炎血清肺表面活性物质及急性时相反应蛋白与LUS的关系。结果:A、B、C组的血清肺表面活性物质表达水平显著低于D组,而血清急性时相反应蛋白表达水平则显著高于D组(P<0.05),且A、B组的上述指标水平显著优于C组,A组的上述指标检测水平显著优于B组,差异均有统计学意义(P<0.05)。相关性分析显示,新生儿肺炎血清肺表面活性物质与LUS呈负相关,血清急性时相反应蛋白与LUS呈正相关(P<0.05)。结论:新生儿肺炎血清肺表面活性物质及急性时相反应蛋白的表达显著异常,且其与肺炎严重程度密切相关,因此在新生儿肺炎中的检测价值较高。 Objective:To investigate and observe the expression change of serum pulmonary surfactant and acute phase response protein of neonatal pneumonia.Method:A total of 150 newborns with pneumonia admitted to Zhongshan Hospital Affiliated to Xiamen University from March 2019 to February 2022 were selected and divided into three groups according to the severity of pneumonia.Severe neonatal pneumonia[lung ultrasound score(LUS)≥18 points]was group A,moderate neonatal pneumonia(LUS 11-17 points)was group B,and mild neonatal pneumonia(LUS≤10 points)was group C,with 50 cases in each group.Another 50 healthy newborns in the same period were selected as group D.Then the expression levels of serum pulmonary surfactant[pulmonary surfactant associated protein-A(SP-A),pulmonary surfactant associated protein-B(SP-B)and pulmonary surfactant associated protein-C(SP-C)]and acute phase response proteins[C-reactive protein(CRP),α_(1)-acid glycoprotein(α_(1)-AG)and ceruloplasmin(CER)]of four groups were detected and compared.The relationship between serum pulmonary surfactant,acute phase reactive protein and LUS of neonatal pneumonia were analyzed.Result:The expression levels of serum pulmonary surfactant in group A,B and C were significantly lower than those in group D,and the expression levels of serum acute phase response protein were significantly higher than those in group D(P<0.05).Moreover,the levels of the above indicators in group A and B were significantly better than those in group C,and the detection levels of the above indicators in group A were significantly better than those in group B,the differences were statistically significant(P<0.05).Correlation analysis showed that serum pulmonary surfactant was negatively correlated with LUS,and serum acute phase reaction protein was positively correlated with LUS(P<0.05).Conclusion:The expression of serum pulmonary surfactant and acute phase response protein of neonatal pneumonia are significantly abnormal,and they have close relationship with the severity of pneumonia,so the
