摘要
目的观察特发性黄斑孔(IMH)患者椭圆体带缺损直径与黄斑区外层视网膜及脉络膜血流面积的相关性。方法前瞻性病例研究。连续纳入山西省眼科医院2019年7月至2021年7月IMH 20例(20只眼), 所有患者行玻璃体切除联合内界膜剥术。比较患者手术前后视力变化, 根据OCT及OCTA检查结果分析手术后视力与椭圆体带缺损直径、黄斑外层视网膜血流面积及脉络膜血流面积的相关性。结果术后6个月:所有患者黄斑孔均闭合;与术前比较, 视力明显提高, 外层视网膜及脉络膜血流面积增大, 椭圆体带缺损直径缩小(F=9.02、16.17、4.18、50.90, 均P<0.001);手术前后视力与椭圆体带缺损直径均呈正相关(r=0.08、0.08, 均P<0.001);术前脉络膜血流面积与外层视网膜血流面积呈正相关(r=0.59, P=0.007), 术前脉络膜血流面积与椭圆体带缺损直径呈负相关(r=-0.49, P=0.027)。结论玻璃体切除治疗IMH可有效促进黄斑孔闭合, 手术前脉络膜血流面积与外层视网膜血流面积呈正相关, 与椭圆体带缺损直径呈负相关。
Objective To observe the correlation between ellipsoid zone defect diameter and blood flow area in outer retina and choroid for idiopathic macular hole(IMH).Methods This was a prospective case study.Twenty eyes of 20 patients with IMH admitted to Shanxi Provice Eye Hospital from Jul.2019 to Jul.2021 were enrolled.All cases received vitrectomy combined with internal limiting membrane peeling.Visual acuity were compared before and after.Application of OCT and OCTA examinations the correlation to vision changes and ellipsoid zone defect diameter,blood flow in outer retina and choroid were analyzed.Results At 6 months after surgery,macular holes of all cases were closed.BCVA significantly improved;blood flow area of outer retina and choroid increased and ellipsoid zone defect diameter reduced compared with before surgery(F=9.02,16.17,4.18,50.90;all P<0.001).There was a positive correlation between visual acuity and ellipsoid zone defect diameter before and after operation(r=0.08,0.08;all P<0.001).There was a positive correlation between the blood flow area of choroid and the outer retina before surgery(r=0.59,P=0.007),and a negative correlation between the blood flow area of choroid and the ellipsoid zone defect diameter(r=-0.49,P=0.027).Conclusion Vitrectomy can treat IMH effectively.The blood flow area of choroid before surgery is positively correlated with the blood flow area of outer retina and negatively correlated with the ellipsoid zone defect diameter.
