摘要
目的量化分析特发性黄斑裂孔(IMH)的脉络膜血循环。方法回顾性分析42例单眼IMH患者,分为患眼组、对侧健眼组,选取对照组。应用相干光层析血管成像术(OCTA)测量受检眼黄斑中心凹下脉络膜厚度(SFCT)和脉络膜毛细血管层血流面积(CBFA),比较其组间差异。结果患眼组、对侧眼健眼、对照组各组平均SFCT分别为(221±67.99)μm、(250.44±57.53)μm、(292.39±56.78)μm,比较分析有统计学差异(F=6.88,P=0.002)。患眼组、对侧健眼组、对照组的CBFA分别为(1.54±0.22)mm^(2)、(1.96±0.21)mm^(2)、(2.01±0.14)mm^(2)。各组CBFA比较,对照组、患眼组和对侧健眼组逐渐减少,但对照组与对侧健眼组间差别无统计学意义。患眼组的CBFA与裂孔直径呈负相关性(P﹤0.05)。结论IMH患眼和对侧健眼SFCT变薄、CBFA减少,引起脉络膜血循环下降,提示脉络膜血循环可能参与了IMH的发生和发展过程。
Objective To quantitatively analyze the choroidal circulation in patients with idiopathic macular hole(IMH).Methods A total of 42 patients with unilateral IMH were retrospectively analyzed and divided into three groups:the affected eye group,unaffected fellow eye group and control group.The subfoveal choroidal thickness(SFCT)and the choriocapillary blood flow area(CBFA)were detected by optical coherence tomography(OCT).The differences in CBFA and SFCT among the three groups were analyzed.Results The mean SFCT in the affected eye group,unaffected fellow eye group and control group was(221±67.99)μm,(250.44±57.53)μm and(292.39±56.78)μm,respectively.And the difference was significant(F=6.88,P=0.002).The mean CBFA in the three groups was(1.54±0.22)mm^(2),(1.96±0.21)mm^(2) and(2.01±0.14)mm^(2),respectively.The CBFA was significantly lower in affected eye group than the unaffected fellow eye group and control group(t=-6.62,P=0.000;t=-7.60,P=0.000,respectively).The CBFA was also lower in the unaffected eye group than the control group,but without significant difference.The CBFA in the affected eye group appeared negatively correlated with the macular hole diameter(P<0.05).Conclusions Both SFCT and CBFA were reduced in both eyes of IMH patients,leading decreased choroidal blood circulation,which suggests that choroidal blood circulation may be involved in the development and progression of IMH.
