摘要
目的 探究体位及上推胎头时机对胎头深陷骨盆转剖宫产术患者取胎头成功率及手术并发症的影响。方法 选取86例因胎头深陷骨盆转剖宫产术的患者,基于SPSS 22.0软件生成的随机分组序列分为对照组与研究组,每组43例。对照组予以平卧位,上推胎头前切开子宫;研究组予以小角度(20°)倾斜头低脚高位,上推胎头后切开子宫。比较两组患者相关指标、不同时段新生儿Apgar评分、取胎头成功率及并发症发生情况。结果 研究组手术时间、胎儿分娩时间及住院时间均短于对照组,术中出血量少于对照组,差异具有统计学意义(P<0.05)。研究组新生儿出生1min与5min的Apgar评分均高于对照组,差异具有统计学意义(P<0.05)。研究组取胎头成功率(100%)高于对照组(90.70%),并发症发生率(4.65%)低于对照组(20.93%),差异具有统计学意义(P<0.05)。结论 针对因胎头深陷骨盆转剖宫产的患者,予以小角度倾斜头低脚高位,上推胎头后切开子宫的方式效果最为理想,其能在促进临床指标的基础上,提升取抬头的成功率,减少并发症的发生。
Objective To explore the effects of body position and timing of pushing up the fetal head on the success rate of fetal head removal and surgical complications in patients undergoing caesarean section with fetal head deep-set pelvis. Methods 86 patients undergoing cesarean section due to fetal head deep-set pelvis were selected and divided into control group and study group based on SPSS 22.0 software random grouping sequence, with 43 cases in each group. The control group was given supine position, and the uterus was cut before pushing up the fetal head. In the study group, the head was tilted at a small Angle(20°) and the foot was high. After pushing the fetal head up, the uterus was cut. The relative indexes, Apgar score, success rate of fetal head removal and complications were compared between the two groups. Results The operation time, fetal delivery time and hospital stay of the study group were shorter than those of the control group, and the intraoperative blood loss was less than that of the control group, the difference was statistically significant(P<0.05). Apgar scores 1 min and 5 min after birth in the study group were higher than those in the control group, and the difference was statistically significant(P<0.05). The success rate of fetal head removal in the study group(100%) was higher than that in the control group(90.70%), and the complication rate(4.65%) was lower than that in the control group(20.93%), the difference was statistically significant(P<0.05). Conclusion For the patients with fetal head deep-seated pelvis cesarean section, the best effect is to tilt the head at a small Angle and lower the foot height, push the fetal head up and cut the uterus, which can improve the success rate of taking the head and reduce the occurrence of complications on the basis of promoting clinical indicators.
