摘要
目的分析低位产钳助产术在头位难产中的应用效果及并发症发生率。方法184例头位难产产妇,均经阴道分娩试产失败中途转用不同方式分娩,依据分娩方式不同分为剖宫产组和产钳组,各92例。剖宫产组采用剖宫产术分娩,产钳组采用低位产钳助产术分娩。对比两组产妇并发症发生情况,新生儿并发症发生情况,临床指标,新生儿出生后1、5、10 min的Apgar评分。结果产钳组产妇并发症发生率42.39%低于剖宫产组的57.61%,差异有统计学意义(P<0.05)。产钳组新生儿并发症发生率19.57%低于剖宫产组的32.61%,差异有统计学意义(P<0.05)。产钳组住院时间(3.43±0.22)d短于剖宫产组的(7.52±0.45)d,治疗费用(4363.24±214.23)元少于剖宫产组的(9000.14±257.73)元,抗生素使用率32.61%低于剖宫产组的100.00%,差异有统计学意义(P<0.05)。出生后1、5、10 min,产钳组新生儿Apgar评分均高于剖宫产组,差异有统计学意义(P<0.05)。结论低位产钳助产术用于产科头位难产分娩产妇,可以减少产妇及新生儿并发症,提升新生儿质量,促进产妇康复,并减少其医疗负担,值得使用。
Objective To analyze the effect and complication rate of low forceps delivery in obstetric delivery.Methods All 184 women with cephalic presentation dystocia were delivered by different methods in the middle of failed vaginal delivery attempts,and were divided into in the cesarean section group and forceps group according to the different delivery methods,with 92 cases in each group.The cesarean section group delivered by cesarean section,and the forceps group delivered by low forceps delivery.The occurrence of maternal complications,occurrence of neonatal complications,clinical indicators,and Apgar scores at 1,5,and 10 min after birth were compared between the two groups.Results The incidence of maternal complications was 42.39%in the forceps group,which was lower than 57.61%in the cesarean section group,and the difference was statistically significant(P<0.05).The incidence of neonatal complications in the forceps group was 19.57%,which was lower than 32.61%in the cesarean section group,and the difference was statistically significant(P<0.05).The length of hospital stay(3.43±0.22)d in the forceps group was shorter than(7.52±0.45)d in the cesarean section group;the cost of treatment(4363.24±214.23)yuan in the forceps group was less than(9000.14±257.73)yuan in the cesarean section group;the antibiotic use rate 32.61%in the forceps group was lower than 100.00%in the cesarean section group;the differences were statistically significant(P<0.05).At 1,5 and 10 min after birth,the Apgar scores in the forceps group were higher than those in the cesarean section group,and the difference was statistically significant(P<0.05).Conclusion Application of low forceps delivery in cephalic presentation dystocia can reduce maternal and neonatal complications,improve neonatal quality,promote maternal rehabilitation,and reduce their medical burden,and is worthy of application.
