摘要
目的研究妊娠早期增重正常,但孕前体质指数(BMI)不同的孕妇的妊娠结局。方法选取2020年1月至2022年1月在战略支援部队特色医学中心妇产科建档产检且分娩,并满足孕早期体重增长在0~2.0kg范围内的677例孕妇的临床资料;(1)参照2021年《中国妇女妊娠期体重监测与评价》标准,根据孕前BMI将研究对象分为;低体重组、正常体重组、超重及肥胖组。(2)记录各组孕妇妊娠期高血压、妊娠期糖尿病、妊娠期贫血、胎膜早破、巨大儿、低出生体重儿、剖宫产、自然分娩的发生率。(3)采用χ^(2)检验、Fisher精确检验、Bonferroni方法对数据进行统计分析。结果孕早期增重正常孕妇中,孕前超重及肥胖组孕妇妊娠期高血压、妊娠期糖尿病发生率均高于低体重组、正常体重组(P<0.05),低体重组的自然分娩率高于正常体重组与超重及肥胖组(P<0.05)。结论孕早期体重、孕前BMI要定期监测,将会有效降低不良妊娠结局的发生。
Objective To study the pregnancy outcomes of pregnant women with normal weight gain in early pregnancy but different pre-pregnancy body mass index(BMI).Methods The clinical data of 677pregnant women who were registered and delivered in the Department of Obstetrics and Gynecology of our center from January 2020to January 2022and met the weight gain in the first trimester within 0-2.0kg were selected.(1)According to the 2021《Chinese Women’s Weight Monitoring and Evaluation during Pregnancy standard》,the pre-pregnancy BMI of the study subjects was divided into two groups according to the pre-pregnancy BMI;Low weight group,normal weight group,overweight and obesity group.(2)The incidence of gestational hypertension,gestational diabetes mellitus,gestational anemia,premature rupture of membranes,macrosomia,low birth weight infants,cesarean section and spontaneous delivery were recorded in each group.(3)χ^(2) test,Fisher exact test and Bonferroni method were used for statistical analysis.Results In pregnant women with normal weight gain in early pregnancy,the incidence of gestational hypertension and gestational diabetes mellitus in overweight and obesity group before pregnancy were higher than those in low weight group and normal weight group(P<0.05),and the natural delivery rate in low weight group was higher than that in normal weight group and overweight and obesity group(P<0.05).Conclusion Attention should be paid not only to the management of weight gain in the first trimester,but also to the management of pre-pregnancy BMI,and early education and intervention should be emphasized,which will effectively reduce the incidence of adverse pregnancy outcomes.
