摘要
目的 探讨血清微小RNA-15b(miRNA-15b)、胰岛素样生长因子-1(IGF-1)在2型糖尿病(T2DM)患者中的变化及与糖尿病视网膜病变(DR)的相关性。方法 选取2020年2月—2021年5月郑州人民医院收治的328例T2DM患者作为研究对象,根据治疗后3个月是否发生DR分为DR组和无DR(NDR)组。统计两组临床资料,比较两组血清miRNA-15b、IGF-1水平及其他可导致DR发生的影响因素差异,采用logistic回归分析分析T2DM患者发生DR的危险因素,采用pareson相关系数模型分析血清miRNA-15b、IGF-1水平与DR发生的相关性。结果 328例T2DM患者,经常规治疗后3个月DR发生率为24.09%(79/328);单因素分析显示,DR组糖化血红蛋白(HbA1c)、空腹血糖(FBG)、miRNA-15b、IGF-1水平均高于NDR组(P<0.05);经多因素logistic回归分析结果显示,FBG、miRNA-15b、IGF-1水平升高为T2DM患者发生DR的独立危险因素(P<0.05);经pareson分析显示,血清miRNA-15b、IGF-1水平与DR发生率均呈正相关关系。结论 血清中miRNA-15b、IGF-1水平升高与T2DM及DR的发生和发展有关,且与DR发生率呈正相关。
Objective To explore the changes of serum microRNA-15 b(miRNA-15 b) and insulin-like growth factor-1(IGF-1) in patients with type 2 diabetes(T2 DM) and their correlation with diabetic retinopathy(DR). Methods Selected 328 cases T2 DM patients admitted to our hospital from February 2020 to May 2021 as the research objects, and divided them into DR group and non-DR(NDR) group according to whether DR occurred 3 months after treatment. The clinical data of the two groups were collected, and the differences in the levels of serum miRNA-15 b, IGF-1 and other factors that could lead to DR were compared between the two groups. Logistic regression analysis was used to analyze the risk factors for DR in T2 DM patients, and the pareson correlation coefficient model was used to analyze the correlation between serum miRNA-15 b and IGF-1 levels and the occurrence of DR. Results In 328 patients with T2 DM,the incidence of DR was 24.09%(79/328) 3 months after conventional treatment;Univariate analysis showed that glycosylated hemoglobin(HbA1 c),fasting blood glucose(FBG),miRNA-15 b, IGF-1 in the DR group The level is higher than the NDR group(P<0.05). The results of multivariate logistic regression analysis showed that elevated levels of FBG,miRNA-15 b, and IGF-1 were independent risk factors for DR in patients with T2 DM(P<0.05). Pareson analysis showed that serum miRNA-15 b and IGF-1 levels were positively correlated with the incidence of DR. Conclusion Increased levels of miRNA-15 b and IGF-1 in serum are related to the occurrence and development of T2 DM and DR,and are positively correlated with the incidence of DR.
