摘要
[目的]探索《局方发挥》(以下简称《发挥》)中朱丹溪对脾胃病的认识。[方法]研读《发挥》,参考《太平惠民和剂局方》(以下简称《局方》)《内经》《伤寒论》《金匮要略》《素问玄机原病式》《脾胃论》等,梳理朱丹溪对《局方》观点的评议和对诸家学术的传承,并概括朱丹溪对脾胃病的认识。[结果]朱丹溪对脾胃病的认识主要有:论辨治,脾病证候未必皆寒,故施治当明辨证候,忌率用温补;论方药,平胃散以微汗之法,治上焦之湿,不能补气,不宜常服;论疾病,泄泻与痢疾有别,二病病机热多寒少,实者宜予承气汤下之,寒者宜予姜、附辈温之;论饮食,辛甘美味,性非中和,多服贻害,损伤脾胃。[结论]朱丹溪对脾胃病的认识特色鲜明,而且颇具现实意义,是研究丹溪学术,指导脾胃病治疗的宝贵资料。
[Objective] To explore ZHU Danxi’s understanding of spleen and stomach diseases in The Development of Jufang(hereinafter referred to as The Development). [Methods] Through studying The Development, referring to Taiping Huimin Heji Jufang(hereinafter referred to as Jufang), Neijing, Treatise on Febrile Diseases, The Synopsis of the Golden Chamber, Plain Questions Mystery Original,Treatise on Spleen and Stomach, etc.,this paper sorts out ZHU Danxi’s comments on the viewpoints of Jufang, the inheritance of the academics of various schools, and summarizes ZHU Danxi’s understanding of spleen and stomach diseases. [Results] ZHU Danxi’s understanding of spleen and stomach diseases mainly includes: In the diagnosis and treatment, the syndromes of spleen diseases are not necessarily all cold, so the treatment should be based on the differentiation of the syndromes, and the use of warming tonic herbs should be careful;in terms of prescriptions, Pingwei powder can slightly sweat to treat the dampness of the upper-Jiao, which cannot invigorate Qi and should not be taken regularly;in terms of diseases, diarrhea is different from dysentery, the pathogenesis of the two diseases is more heat and less cold, for those who have the excess syndrome, Chengqi Decoction should be used according to purgative method;for those who have the cold syndrome, Chinese herbs such as dried ginger and aconite should be used according to warm method;in terms of diet, for the sweet and delicious kind, its nature is not neutral, overeating is harmful and it will harm the spleen and stomach.[Conclusion] ZHU Danxi’s understanding of spleen and stomach diseases has distinctive features and practical significance. It is a valuable material for studying Danxi academics and guiding the treatment of spleen and stomach diseases.
