摘要
目的:探讨半夏白术天麻汤合丹参饮对痰瘀型经皮冠状动脉介入(PCI)术后心绞痛患者临床疗效及相关血清学指标水平的影响。方法:纳入126例痰瘀型PCI术后心绞痛患者,随机分为对照组及观察组各63例。对照组给予双联抗血小板聚集、降脂、控制心率、预防心肌重塑等药物治疗;观察组在对照组常规西药治疗的基础上,加用半夏白术天麻汤合丹参饮治疗,疗程3个月。比较两组患者临床总有效率以及治疗前后中医证候积分、营养代谢相关指标和炎症因子水平。结果:经3个月治疗,对照组和观察组患者的临床总有效率分别是73.02%、90.00%,两组疗效比较差异有统计学意义(P<0.05)。同组患者治疗前后比较,中医证候积分均明显下降(P<0.01),前白蛋白(PAB)水平明显升高(P<0.05),同型半胱氨酸(Hcy)、视黄醇结合蛋白(RBP)和低密度脂蛋白(LDL)水平明显下降(P<0.05),白细胞介素-6(IL-6)、超敏C反应蛋白(hs-CRP)以及肿瘤坏死因子-α(TNF-α)水平呈现明显下降趋势(P<0.01)。与对照组比较,治疗后观察组中医证候积分低于对照组(P<0.01),PAB水平高于对照组(P<0.05),Hcy、RBP和LDL水平低于对照组(P<0.05),炎症因子IL-6、hs-CRP、TNF-α水平均低于对照组(P<0.01)。结论:半夏白术天麻汤合丹参饮可明显提高痰瘀型PCI术后心绞痛患者临床治疗的总有效率,改善临床症状,可能与其减轻炎症反应、调控营养物质的代谢、调节血脂水平、改善冠脉微循环有关。
Objective:To investigate the clinical effect of Banxia Baizhu Tianma Decoction(半夏白术天麻汤)and Danshen Decoction(丹参饮)for phlegm and blood stasis type angina pectoris after primary coronary intervention(PCI).Methods:Total 126 patients with angina pectoris after PCI of phlegm and blood stasis type were randomly divided into control group and observation group,with 63 cases in each group.The control group was treated with dual antiplatelet aggregation,lipid-lowering,heart rate control and myocardial remodeling prevention.On the basis of routine western medicine treatment in the control group,the observation group was treated with Banxia Baizhu Tianma Decoction and Danshen Decoction for 3 months.The total effective rate,traditional Chinese medicine(TCM)symptom score,nutritional metabolism related indexes and inflammatory factor levels before and after treatment were compared between the two groups.Results:After 3 months of treatment,the total effective rates of the control group and the observation group were 73.02%and 90.00%.The difference between the two groups was statistically significant(P<0.05).Compared with the patients in the same group before and after treatment,the TCM syndrome score decreased significantly(P<0.01);prealbumin(PAB)level increased significantly(P<0.05);homocysteine(Hcy),retinol binding protein(RBP)and low density lipoprotein(LDL)levels decreased significantly(P<0.05);interleukin-6(IL-6),high sensitivity C-reactive protein(hs-CRP)and tumor necrosis factor-α(TNF-α)showed a significant downward trend(P<0.01).Compared with the control group,the TCM syndrome score of the observation group after treatment was significantly lower(P<0.01);PAB level was significantly higher than the control group(P<0.05);Hcy,RBP and LDL levels were significantly lower than the control group(P<0.05);inflammatory factors IL-6,hs-CRP and TNF-αwere significantly lower than the control group(P<0.01).Conclusions:Banxia Baizhu Tianma Decoction and Danshen Decoction can significantly improve the total effec
