Th1/Th2细胞和25(OH)D的交互关系及其在妊娠期糖尿病发病中的作用 被引量:3

摘要 目的 探讨Th1/Th2细胞和25(OH)D的交互关系及其在妊娠期糖尿病(gestational diabetes mellitus,GDM)发病中的作用。方法 前瞻性纳入2019年6月至2021年12月医院收治的GDM患者48例作为研究对象(GDM组),另选取同期于医院行常规产检的48例正常妊娠孕妇作为对照组(GNGT组)。应用流式细胞术对2组患者外周血中Th1/Th2细胞比例进行分析,并采用ELISA法检测其血清白细胞介素-2(interleukin-2,IL-2)、IL-10、IL-4及干扰素-γ(interferon-γ,IFN-γ)水平,电化学发光法检测外周血中25(OH)D水平,通过Pearson相关性分析法分析25(OH)D与Th1/Th2细胞之间的相关性。结果 GDM组患者空腹血糖(fasting plasma glucose,FPG)、2 h FPG、空腹胰岛素(fasting insulin,FINS)及胰岛素抵抗指数(insulin resistance index,HOMA-IR)值均较GNGT组明显偏高(P<0.05)。GDM组患者血清IL-2、IFN-γ水平高于GNGT组,IL-4、IL-10水平较对照组显著偏低,均有统计学差异(P<0.05)。GDM组Th1细胞比例及Th1/Th2值较GNGT组显著偏高,Th2细胞比例低于GNGT组(P<0.05)。GDM组患者血清25(OH)D水平明显低于GNGT组(P<0.05)。Pearson分析结果显示,25(OD)H与FPG、2 h FPG、FINS、HOMA-IR等临床生化指标及IL-2、IFN-γ水平呈负相关(P<0.05),与IL-4、IL-10水平呈正相关(P<0.05)。多元线性回归分析结果显示,Th1、Th2细胞比例是GDM患者外周血25(OH)D水平的影响因素(P<0.05)。结论 GDM孕妇Th1/Th2细胞比例失衡可能与低血清25(OH)D水平的调节作用相关。 In order to analyze the interaction between th1/th2 cells and 25(OH)D and its role in the pathogenesis of gestational diabetes(GDM), 48 patients with GDM were selected as GDM group, and 48 normal pregnant women undergoing routine prenatal examination were selected as gestational normal glucose tolerance(GNGT) group. Flow cytometry was used to detect the ratio of Th1/Th2 cells in peripheral blood, while ELISA was used to detect the levels of interleukin-2(IL-2), IL-10, IL-4 and interferon-γ(IFN-γ) in serum. And the correlation between 25(OH)D and Th1/Th2 cells was explored. Data showed that the imbalance of Th1/Th2 cell ratio and immune dysfunction were common in pregnant women with GDM. Furthermore, 25(OH)D was negatively related with the levels of fasting plasma glucose(FPG), 2 h FPG, fasting insulin(FINS) and insulin resistance index(HOMA-IR), but positively related with the levels of IL-4 and IL-10. In conclusion, the imbalance of Th1/Th2 cell ratio and immune dysfunction in GDM pregnant womenmay related to the regulation of low serum 25(OH)D level.
