摘要
目的 对妊娠期糖尿病(gestational diabetes mellitus,GDM)患者实施二甲双胍联合胰岛素治疗对其妊娠结局及临床疗效产生的影响展开分析与探讨。方法 选取2019年4月—2021年8月期间福建省妇幼保健院收治的80例GDM患者为研究对象,按随机数字表法等分为两组,对照组40例应用常规胰岛素治疗;研究组40例应用二甲双胍联合胰岛素治疗,观察比较临床疗效。结果 治疗后,研究组空腹血糖(5.80±1.03)mmol/L、餐后2 h血糖(7.74±1.49)mmol/L、糖化血红蛋白(5.44±1.10)%均低于对照组,差异有统计学意义(t=2.694、2.280、4.245,P<0.05);研究组不良妊娠结局发生率(10.00%)低于对照组,差异有统计学意义(χ^(2)=4.020,P<0.05);研究组不良围生儿结局发生率(7.50%)低于对照组,差异有统计学意义(χ^(2)=4.500,P<0.05);治疗后,研究组同型半胱氨酸(8.24±2.19)μmol/L、血清胱抑素(0.71±0.20)mg/L均低于对照组,差异有统计学意义(t=3.401、7.677,P<0.05)。结论 使用二甲双胍联合胰岛素治疗方案有助于改善GDM疾病人群的妊娠结局,总体效果突出。
Objective To analyze and investigate the effects of metformin combined with insulin treatment on pregnancy outcomes and clinical efficacy in patients with gestational diabetes mellitus(GDM).Methods 80 GDM patients admitted to Fujian Provincial Maternal and Child Health Hospital from April 2019 to August 2021 were selected as research objects. Two groups were equally divided according to the random number table method, and 40 cases in the control group were treated with conventional insulin. In the study group, 40 cases in the study group applied metformin combined with insulin treatment to observe and compare the clinical efficacy.Results After treatment, fasting blood glucose(5.80±1.03) mmol/L, 2 h postprandial blood glucose(7.74±1.49) mmol/L, glycosylated hemoglobin(5.44±1.10)% in study group were lower than those in control group, and the difference was statistically significant(t=2.694, 2.280, 4.245, P<0.05). The incidence of adverse pregnancy outcomes in the study group was 10.00% lower than that in the control group, and the difference was statistically significant(χ^(2)=4.020, P<0.05). The incidence of adverse perinatal outcomes in the study group was 7.50% lower than that in the control group, and the difference was statistically significant(χ^(2)=4.500, P<0.05);after treatment, homocysteine(8.24±2.19) μmol/L and serum cystatin(0.71±0.20) mg/L in study group were lower than those in control group, and the difference was statistically significant(t=3.401, 7.677, P<0.05).Conclusion The use of metformin combined with insulin treatment regimen helps to improve pregnancy outcome in GDM disease population with outstanding overall effect.
