青蒿琥酯对HAART后免疫重建不良HIV/AIDS患者免疫功能和肠道菌群影响的研究 被引量:3

Effects of Artesunate on Immune Function and Gut Microbiota in HIV/AIDS Patients with Poor Immune Reconstitution after HAART

摘要 目的:探索青蒿琥酯对接受高效抗逆转录病毒疗法(HAART)后免疫重建不良的艾滋病病毒(HIV)感染者/艾滋病(AIDS)患者(简称HIV/AIDS患者)免疫功能及肠道菌群的影响。方法:采用前后自身对照的研究方法,流式细胞仪检测CD4^(+)T、CD4^(+)CD45RA^(+)T、CD4^(+)CD45RO^(+)T、CD8^(+)T、CD4^(+)CD38^(+)/CD3^(+)、CD4^(+)CD28^(+)/CD3^(+)、CD4^(+)CD25^(+)/CD3^(+)、CD8^(+)CD38^(+)/CD3^(+)、CD4^(+)Ki67/CD4^(+)、CD4^(+)Ki67/CD8^(+)、早期凋亡细胞/CD3^(+)、早期凋亡细胞/CD4^(+)、早期凋亡细胞/CD8^(+);16S rDNA高通量测序技术检测粪便菌群多样性、物种数量与群落组成及物种丰度差异。结果:与基线相比,服用青蒿琥酯24周后CD4^(+)、CD4^(+)CD45RA^(+)、CD4^(+)CD45RO^(+)、CD8^(+)T淋巴细胞均有所上升,但差异无统计学意义(P>0.05);服用青蒿琥酯24周后CD4^(+)CD38^(+)/CD3^(+)、CD4^(+)CD28^(+)/CD3^(+)、CD4^(+)CD25^(+)/CD3^(+)、CD8^(+)CD38^(+)/CD3^(+)、CD4^(+)Ki67/CD4^(+)、CD4^(+)Ki67/CD8^(+)、早期凋亡细胞/CD3^(+)、早期凋亡细胞/CD4^(+)、早期凋亡细胞/CD8^(+)下降,其中早期凋亡细胞/CD4^(+)明显下降(P<0.01);服用青蒿琥酯24周后各多样性指数和菌落结构均未见显著差异,疣菌门显著增多(P<0.05)。结论:青蒿琥酯片对HAART后免疫重建不良患者以50 mg/d,每日2次的剂量干预24周后,有效降低了CD4^(+)T淋巴细胞早期凋亡的水平;另外,青蒿琥酯可能会带来肠道共生菌疣菌门(Verrucomicrobiota)的增多。 Objective:To explore the effects of artesunate on immune function and gut microbiota in HIV/AIDS patients with poor immune reconstitution after antiviral therapy(HAART).Methods:The method of self-control before and after was adopted.Flow cytometry was used to detect CD4^(+)T,CD4^(+)CD45RA^(+)T,CD4^(+)CD45RO^(+)T,CD8^(+)T,CD4^(+)CD38^(+)/CD3^(+),CD4^(+)CD28^(+)/CD3^(+),CD4^(+)CD25^(+)/CD3^(+),CD8^(+)CD38^(+)/CD3^(+),CD4^(+)Ki67/CD4^(+),CD4^(+)Ki67/CD8^(+),early apoptotic cells/CD3^(+),early apoptotic cells/CD4^(+),and early apoptotic cells/CD8^(+).Gut microbiota diversity,differences in species number,community composition and species abundance were detected by 16S rDNA high-throughput sequencing technology.Results:CD4^(+),CD4^(+)CD45RA^(+),CD4^(+)CD45RO^(+)and CD8^(+)T lymphocytes increased after 24 weeks of artesunate treatment compared with those of the baseline,but there was no statistical difference(P>0.05).After 24 weeks of artesunate administration,CD4^(+)CD38^(+)/CD3^(+),CD4^(+)CD28^(+)/CD3^(+),CD4^(+)CD25^(+)/CD3^(+),CD8^(+)CD38^(+)/CD3^(+),CD4^(+)Ki67/CD4^(+),CD4^(+)Ki67/CD8^(+),early apoptotic cells/CD3^(+),early apoptotic cells/CD4 and early apoptotic cells/CD8^(+)decreased,and early apoptotic cells/CD4^(+)decreased most statistically(P<0.01).There were no significant differences in diversity index and colony structure after 24 weeks of artesunate administration,and there was a statistically significant increase in the number of P_verrucomicrobiota(P<0.05).Conclusion:Artesunate Tablet can effectively reduce the level of early apoptosis of CD4^(+)T lymphocytes after 24 weeks of treatment with 50 mg,2 times,per day in patients with poor immune reconstruction after HAART.In addition,artesunate may lead to an increase in the intestinal symbiotic bacterium of P_verrucomicrobiota.
