摘要
目的分析初治病原学阳性且利福平敏感肺结核患者不同阶段T细胞斑点试验(T-SPOT.TB)、结核抗体与痰涂片+培养检测动态变化及临床意义。方法收集2020年8月至2021年5月北京结核病控制研究所门诊收治的肺结核患者34例,分别于治疗前、治疗2月和6月行以上方法检测。结果34例患者,疗前痰涂片+培养、T-SPOT.TB、结核抗体阳性检出率分别为85.29%、100%、76.47%;治疗2月阴转率分别为89.66%、5.88%、11.54%;治疗6月阴转率分别为100%、23.53%、26.92%。治疗前、治疗2月和6月之间T-SPOT.TB试验结核特异性抗原与植物血凝素(TBAg/PHA)比值差异均有统计学意义(P值均<0.05),治疗前与治疗2月或6月痰涂片+培养差异有统计学意义(P值均<0.05),结核抗体差异均无统计学意义(P值均>0.05)。结论在有效抗结核治疗6月后,肺结核患者的T-SPOT.TB试验TBAg/PHA比值显著降低;结核抗体不能很好地反映疗效情况;痰涂片+培养亦能够很好地反映疗效。
Objective To analyze the dynamic changes and clinical significance of T-SPOT test(T-SPOT.TB),tuberculosis antibody,and sputum smear+culture in pulmonary tuberculosis patients with positive etiology and rifampicin sensitivity.Methods A total of 34 patients with pulmonary tuberculosis admitted to the outpatient department of the Beijing Institute of Tuberculosis Control from August 2020 to May 2021 were collected.The above methods were tested before treatment,2 months,and 6 months after treatment.Results Among the 34 patients,the positive detection rates of sputum smear and culture,T-SPOT.TB,tuberculosis antibody before treatment were 85.29%,100%,76.47%,respectively.After 2 months of treatment,the rate of negative conversion was 89.66%,5.88%,and 11.54%,respectively.After 6 months of treatment,the rate of negative conversion was 100%,23.53%,and 26.92%,respectively.There were significant differences in the ratio of TBAg/PHA in T-SPOT.TB test before treatment and between 2 months and 6 months of treatment(all P<0.05).There were significant differences in sputum smear+culture between before treatment and 2 months or 6 months of treatment(all P<0.05).There was no significant difference in tuberculosis antibodies(all P>0.05).Conclusion After 6 months of effective antituberculosis treatment,the ratio of TBAg/PHA in the T-SPOT.TB test was significantly reduced in patients with tuberculosis.Tuberculosis antibody is not a good indicator of efficacy.Sputum smear+culture can also reflect the curative effect.
