摘要
目的探讨血清淀粉样蛋白A(SAA)、单核细胞趋化蛋白-1(MCP-1)在糖尿病肾病(DN)患者中的表达情况,并分析血清SAA、MCP-1水平与DN患者糖脂代谢的关系。方法回顾性分析,收集南阳医专一附院2018年10月至2020年10月收治的86例DN患者的临床资料纳入DN组,另收集同期该院收治的80例糖尿病患者的临床资料纳入糖尿病组。采用免疫散射比浊法测定血清SAA水平、采用酶联免疫吸附法测定血清MCP-1水平,并采用全自动生化分析仪测定糖脂代谢[空腹血糖(FPG)、餐后2 h血糖、总胆固醇(TC)、甘油三酯(TG)]指标。采用一般双变量Peason相关性分析血清SAA、MCP-1水平与糖脂代谢的关系。结果DN组SAA、MCP-1、FPG、餐后2 h血糖、TC、TG水平均高于糖尿病组,差异有统计学意义(P<0.05);Ⅳ期DN患者血清SAA、MCP-1、糖脂代谢指标(FPG、餐后2 h血糖、TC、TG)最高,其次为Ⅲ期、Ⅱ期,Ⅰ期患者血清SAA、MCP-1及糖脂代谢指标最低,4组间比较,差异有统计学意义(P<0.05);经一般双变量Peason相关性分析证实,DN患者血清SAA、MCP-1水平与糖脂代谢指标(FPG、餐后2 h血糖、TC、TG)呈正相关(r>0,P<0.05)。结论血清SAA、MCP-1水平在DN患者中呈异常高表达情况,且其与DN患者糖脂代谢呈正相关。
Objective To investigate the expression of serum amyloid A(SAA)and monocyte chemoattractant protein-1(MCP-1)in patients with diabetes nephropathy(DN),and to analyze the relationship between serum SAA,MCP-1 and glucose and lipid metabolism in patients with DN.Methods Retrospective analysis was conducted to collect the clinical data of 86 patients with DN admitted to the First Affiliated Hospital of Nanyang Medical College from October 2018 to October 2020,and included in the DN group,while the clinical data of 80 patients with diabetes treated in this hospital during the same period were collected,and included in the diabetes group.The serum SAA level was measured by immune scattering turbidimetry,the serum MCP-1 level was measured by enzyme-linked immunosorbent assay,and the indexes of glucose and lipid metabolism[fasting blood glucose(FPG),2 h postprandial blood glucose,total cholesterol(TC),triglyceride(TG)]were measured by automatic biochemical analyzer.The relationship between serum SAA,MCP-1 levels and glucose and lipid metabolism were analyzed by general bivariate Pearson correlation.Results The levels of SAA,MCP-1,FPG,2 h postprandial blood glucose,TC and TG in DN group were higher than those in diabetes group,the difference was statistically significant(P<0.05).Serum SAA,MCP-1 and glucose and lipid metabolism indexes(FPG,2 h postprandial blood glucose,TC and TG)were the highest in patients with stage IV DN,followed by stageⅢandⅡ.Serum SAA,MCP-1 and glucose and lipid metabolism indexes were the lowest in patients with stage I DN,there was statistical significant difference among the four groups(P<0.05).The general bivariate Pearson correlation analysis showed that the levels of serum SAA and MCP-1 in patients with DN were positively correlated with the indexes of glucose and lipid metabolism(FPG,2 h postprandial blood glucose,TC and TG)(r>0,P<0.05).Conclusion The levels of serum SAA and MCP-1 are abnormally high in patients with DN,and they are positively correlated with glucose and lipid metabolism in
