摘要
目的 观察sKL蛋白在不同C肽水平2型糖尿病(T2DM)人群中的表达水平及与胰岛素抵抗之间相关性。方法 选取T2DM患者205例,根据空腹及口服糖耐量试验(OGTT) 2 h C肽水平分为高C肽血症组93例、低C肽血症组112例及正常对照组115例。比较各组血清sKL蛋白水平并分析其与不同C肽水平T2DM人群与胰岛素抵抗的相关性。结果 T2DM伴高C肽血症及低C肽血症患者的血清sKL蛋白低于正常对照组[分别为(392.57±55.61)pg/mL vs.(424.40±76.34)pg/mL vs.(654.79±103.08)pg/mL,P<0.05],尤其以T2DM伴高C肽血症组下降最为显著。相反,T2DM伴高C肽血症及低C肽血症的胰岛素抵抗指数高于正常对照组[14.38±5.25 vs. 1.61±0.93 vs. 0.94±0.08,P<0.05],尤其以T2DM伴高C肽血症升高最为显著。此外,T2DM伴高C肽血症及低C肽血症sKL蛋白水平与胰岛素抵抗指数之间存在负相关,且在T2DM伴高C肽血症患者相关性强。最后通过受试者工作特征(ROC)曲线发现血清sKL蛋白在区分T2DM伴高C肽血症、低C肽血症与正常对照之间有较好表现,尽管在T2DM伴高C肽血症与低C肽血症患者之间不具有显著性差异。结论 T2DM伴高C肽血症及低C肽血症T2DM患者血清sKL蛋白水平降低,其水平与胰岛素抵抗指数呈负相关,尤其在T2DM伴C肽水平升高的T2DM患者中sKL蛋白可能是一个反映患者胰岛素抵抗及T2DM进展的监测指标。
Objective To elucidate the association between serum Klotho and insulin resistance(IR)in type 2 diabetes mellitus(T2DM)patients with differential patterns of C-peptide status.Methods 205 patients with T2DM were enrolled in the study.Participants were classified according to the degree of serum C-peptide,including 115 healthy controls,93 T2DM with hyper-C-peptidaemia,and 112 T2DM with hypo-C-peptidaemia.The association between the serum sKL levels and IR was analyzed under differential patterns of C-peptide status.Results Klotho levels were significantly down-regulated in T2DM with C-peptide disorders compared to controls[(392.57±55.61)pg/mL vs.(424.40±76.34)pg/mL vs.(654.79±103.08)pg/mL,P<0.05],which was most prominent in hyper-C-peptidaemia group.On the contrary,HOMA-IR levels were significantly elevated in case groups compared with controls(14.38±5.25 vs.1.61±0.93 vs.0.94±0.08,P<0.05),which was most prominent in hyper-C-peptidaemia group.In addition,Klotho was negatively associated with HOMA-IR in T2DM with C-peptide disorders,especially in T2DM with hyper-C-peptidaemia.Klothocan differentiating T2DM patients with C-peptidaemia disorders.Conclusion In T2DM,serum Klotho is negatively correlated with HOMA-IR and could be a useful marker for HOMA-IR screening and progression in T2DM with hyper-C-peptide disorders.
