摘要
目的观察喙肩韧带(CAL)的形态特征,为肩袖损伤、肩关节减压的外科治疗提供解剖学依据。方法100例(左侧60例、右侧40例)结构完整的人肩关节标本,观察CAL的形态并分类;分别测量CAL喙突端、肩峰端宽度,内侧缘及外侧缘的长度。结果(1)CAL的形态及数量:带状19例;四边形15例;Y字形19例;V字形31例;多条带形(条带数≥3)16例;(2)多条带形喙突端宽度为(30.82±4.11)mm,显著高于其他类型(F=25.56,P<0.01);Y字形内侧缘长度为(33.08±5.96)mm,明显高于其他类型(F=2.83,P=0.029)。结论对CAL的定量研究及分型,不仅是对既往研究的补充,也为其影像学研究奠定基础。CAL的变异会影响肩峰⁃喙突之间空间构筑,引起关节及软组织损伤。
Objective The study was designed to explore the morphometry of the coracoacromial ligament(CAL) in the glenohumeral joint. Methods A total of 100 glenohumeral joints were included in this study. Types of the CAL were classified according to their morphologies. Widths of the coracoid and acromial attachments,lengths of the lateral and medial borders were measured with a digital venier caliper. Results(1)The CAL occurred in:band-shaped in 19 cases;quadrangular in 15 cases;Y-shaped in 19 cases;V-shaped in 31 cases and multiple bands in 16 cases.(2)Length of the coracoid attachment of the multiple bands was(30.82±4.11)mm,which was longer than any other types(F=25.56,P<0.01). There was statistical significance in the medial borders between Y-shaped and other types(F=2.83,P=0.029). Conclusion The CAL plays an important role in glenohumeral joint stability and proprioception. The study emphasizes the variants of the CALs,and this essential information reaffirms the importance of imaging examination before surgical procedures.
