摘要
目的:回顾性调查武汉大学中南医院感染科2015年1月1日—2019年10月31日感染患者各类标本真菌的分离培养。方法:标本接种于沙保弱培养基进行培养,采用科马嘉念珠菌显色培养基(CHRO⁃Magar)和/或全自动微生物快速检测质谱仪结合形态学进行菌种鉴定。结果:各年度真菌阳性率依次为4.91%、2.47%、3.12%、2.75%以及4.04%。5年培养出的阳性菌株共计313株。其中,呼吸道标本(包括痰和灌洗液)分离出125株;血液培养分离出77株、骨髓标本52株;清洁中段尿标本30株;脑脊液29株。以白色假丝酵母菌、马尔尼菲蓝状菌和新生隐球菌三种为主,占所有分离真菌的79%以上。结论:本院感染科真菌感染以呼吸系统、中枢神经系统、泌尿系和循环系统为主。菌株分布以白色假丝酵母菌、马尔尼菲蓝状菌和新生隐球菌为主,同时还有其他假丝酵母菌、曲霉菌和毛霉菌等。
Objective:To retrospectively investigate the fungal isolations from various specimens of infected patients from January 1,2015 to October 31,2019 in the Infectious Department of Zhongnan Hospital of Wuhan University.Methods:Specimens were inoculated into Sabouraud′s medium to culture,and strains were identified by CHRO⁃Magar and/or full⁃automatic microbial rapid detection mass spec⁃trometer combined with morphology.Results:The positive rates of fungi in years 2015⁃2019 were 4.91%,2.47%,3.12%,2.75%,and 4.04%respectively.A total of 313 positive strains were iso⁃lated in 5 years.Among them,125 strains were isolated from respiratory tract specimens(including sputums and lavage fluids).Among them,77 strains were from bone marrows,52 were from blood cultures,30 were from urine samples,and 29 from cerebrospinal fluids.Candida albicans,Talaromyces marneffei,and Cryptococcus neoformans were the three main species,accounting for more than 79%of all isolated fungi.Conclusion:The sites of fungal infection of the in⁃hospital patients were mainly respiratory tract,central nervous system,urinary system,and circulatory system.The strains mainly included Candida albicans,Talaromyces Marneffei,and Cryptococcus neoformans,as well as other Candida,Aspergillus,and Mucor.
