糖尿病足感染病原菌耐药性及相关危险因素分析 被引量:9

Pathogenic bacteria causing diabetic foot infections and analysis of risk factors

摘要 目的探讨糖尿病足感染病原菌的分布、类型及构成,并对主要病原菌的耐药性进行分析,初步探讨其危险因素,为临床预防和治疗糖尿病足提供指导。方法选取2010年-2012年10月糖尿病足感染患者80例送检标本进行分离培养,对分离出的病原菌分布、类型进行分析,采用K-B琼脂法对主要病原菌进行药敏试验。结果80例糖尿病足患者的分泌物、血液、粪便及其他标本经分离共培养出病原菌132株,其中分泌物中检出76株占57.58%,血液和粪便中分别占19.70%和13.64%,其他占9.09%;其中革兰阳性菌76株占57.58%,革兰阴性菌56株占42.42%;革兰阳性菌对乙酰唑胺和万古霉素的耐药率较低为0~3.45%;革兰阴性菌对地贝卡星、比阿培南和帕尼培南的耐药率较低为0~7.14%。结论糖尿病足感染患者的病原菌种类繁多,且对主要抗菌药物均具有一定的耐药率,临床治疗糖尿病足感染患者时,为减少抗菌药物耐药性,应及时反复细菌培养和药敏试验,严格根据药敏试验结果,合理使用抗菌药物。 OBJECTIVE To investigate the distribution of species of pathogenic bacteria causing diabetic foot infec- tions and analyze the drug resistance of the main pathogens and explore the risk factors so as to guide the clinical prevention and treatment of diabetic foot. METHODS The specimens obtained from 80 cases of patients with diabetic foot infections in the past two years were cultured, then the distribution of species of the isolated patho- gens were analyzed, and the drug susceptibility testing for the main pathogens was performed. RESULTS Totally 132 strains of pathogens were isolated from the secretions, blood, feces, or other specimens which were obtained from 80 cases of diabetic foot patients, among which there were 76 (57.58%) strains isolated from the secretions, 19.70% isolated from the blood, 13.64% isolated from feces, and 9.09% isolated from other specimens% there were 76 (57.58%) strains of gram-positive bacteria and 56 (42.42%) strains of gram-negative bacteria. The drug resistance rate of the gram-positive bacteria to vancomycin or acetazolamide varied between 0 and 3.45 % ; the drug resistance rate of the gram-negative bacteria to dibekacin, biapenem or varied between 0 and 7. 14%. CONCLUSION The species of pathogens causing the diabetic foot infections are varied and have certain drug resistance to the main antibiotics. For the clinical treatment of the patients with diabetic foot infections, it is necessary to perform the repeated bacterial culture as well as the drug susceptibility testing and to reasonably use antibiotic based on the result of drug susceptibility testing so as to reduce the drug resistance.
