摘要
目的:基于文献研究及专家问卷探索尘肺病证素分布及组合规律、证候分布规律及临床特征。方法:检索中国知网数据库、万方数据知识服务平台、中国科技期刊全文数据库、中国生物医学文献数据库已发表的尘肺病证候相关文献,提取信息,规范证候、证素等名词术语,统计分析尘肺病证素分布及组合规律、证候分布规律;在文献研究的基础上开展专家问卷,分析证候分布规律及其临床特征。结果:共纳入合格文献48篇,共163条记录;经规范后的证候共34个,其中肺气虚证、肺阴虚证、燥邪犯肺证、痰湿阻肺证、肺脾气虚证频次较高;病性证素以气虚、阴虚、痰为主,病位证素以肺、肾、脾为主;证素组合形式以两证素组合及三证素组合多见。专家问卷回收23份,专家积极系数92.0%,专家权威程度系数0.86,专家协调系数0.242;肺气虚证、痰瘀阻肺证、肺气阴两虚证和肺肾气阴两虚证的均值、满分比较高,变异系数小,并对各个证候临床特征进行初步分析。结论:尘肺病证候以肺气虚证、肺阴虚证、痰湿阻肺证、肺脾气虚证、肺气阴两虚证、肺肾气阴两虚证较为多见,为进一步开展尘肺病证候诊断标准临床调查研究提供依据。
Objective:To explore the distribution and combination of syndrome elements,syndrome distribution,and clinical characteristics of pneumoconiosis based on literature research and expert questionnaires.Methods:The published literatures related to pneumoconiosis syndromes in the CNKI database,Wanfang data knowledge platform,Chinese scientific journal full-text database,and Chinese biomedical literature database were searched,and the information was extracted to standardize terms such as syndromes and syndromes elements.Statistical analysis of the distribution of pneumoconiosis syndrome elements and their combination,as well as the distribution of syndromes.Based on literature research,an expert questionnaire was carried out to analyze the distribution law of syndromes and their clinical characteristics.Results:A total of 48 qualified literatures were included,with a total of 163 records.After standardization,34 syndromes were obtained.Among them,the frequency of Lung Qi deficiency syndrome,Lung Yin deficiency syndrome,dryness invading the Lung syndrome,phlegm-dampness blocking Lung syndrome,and Lung Spleen Qi deficiency syndrome are relatively high.The syndrome elements of disease nature are mainly Qi deficiency,Yin deficiency,and phlegm,and the syndrome elements of disease location are mainly Lung,Kidney,and Spleen.The combination of syndrome elements is more common in the combination of two syndromes and three syndromes.A total of 23 expert questionnaires were returned,with an expert positive coefficient of 92.0%,expert authority coefficient of 0.86,and expert coordination coefficient of 0.242.The mean value and full score of Lung Qi deficiency syndrome,Phlegm and Blood stasis obstructing Lung syndrome,Lung Qi Yin deficiency syndrome,and Lung Kidney Qi Yin deficiency syndrome are relatively high,and the coefficient of variation is small,and the clinical characteristics of each syndrome are preliminarily analyzed.Conclusion:Pneumoconiosis syndromes are more common in Lung Qi deficiency,Lung Yin deficiency,Phlegm and
