摘要
肺动脉高压心力衰竭在临床中不仅病情进展迅猛,易反复发作,根治困难;还具有高死亡率、低存活率的特点,尤其是危重肺动脉高压心力衰竭更为明显。有效治疗肺动脉高压心力衰竭对提高患者存活率、延长其生存期、提高其生活质量具有重要意义。肺动脉高压心力衰竭同时具备心力衰竭与肺动脉高压的特点及临床表现,但是临床上以心力衰竭肺动脉高压更多见、广泛、普遍,从而导致肺动脉高压心力衰竭的治疗原则与心力衰竭肺动脉高压相近,甚至有肺动脉高压心力衰竭被误诊为心力衰竭肺动脉高压,使肺动脉高压心力衰竭在临床治疗中存在严重的局限性。本文通过对2例危重肺动脉高压心力衰竭临床治疗资料的收集与整理提出“早期干预、心肺同治”治疗肺动脉高压心力衰竭的原则,同时也为拓展肺动脉高压心力衰竭治疗思维与治疗方案提供文献支持。
Pulmonary arterial hypertension in the clinical heart failure is not only rapid progress,easy to relapse,radical treatment is difficult;It also has the characteristics of high mortality and low survival rate,especially the critical pulmonary hypertension heart failure is more obvious.Effective treatment of heart failure with pulmonary arterial hypertension is of great significance for improving survival rate,prolonging survival period and improving quality of life of patients.Pulmonary hypertension heart failure has the characteristics and clinical manifestations of both heart failure and pulmonary hypertension,but pulmonary hypertension in heart failure is more common,extensive and common in clinic,which leads to the treatment principle of pulmonary hypertension heart failure is similar to that of heart failure pulmonary hypertension,and even some pulmonary hypertension heart failure is misdiagnosed as heart failure pulmonary hypertension.There are serious limitations in the clinical treatment of heart failure with pulmonary arterial hypertension.Based on the collection and collation of clinical treatment data of 2 patients with critical pulmonary hypertension heart failure,this paper proposes the principle of“early intervention and cardiopulmonary treatment”for the treatment of pulmonary hypertension heart failure,and also provides literature support for expanding the treatment thinking and treatment plan of pulmonary hypertension heart failure.
