摘要
目的 观察分析不同缝合方式对首次剖宫产产妇子宫下段肌层愈合及子宫切口瘢痕憩室(previouscesareanscardefect,PCSD)形成的影响。方法 回顾性收集2020年1月—2022年1月于潮州市人民医院行首次剖宫产手术的300例产妇为研究对象,依据剖宫产术后切口不同缝合方式将其分为两组,各150例。对照组采用单层缝合方式,观察组采用双层缝合方式,对比观察两组产妇手术相关指标,术后随访时间≥6个月,对比两组患者术后恶露持续时间、月经紊乱发生率、切口愈合不良发生率及PCSD的发生情况。结果 观察组产妇手术时间明显长于对照组[(25.38±5.38)min vs.(22.31±6.03) min],切口愈合不良发生率显著低于对照组(3.33%vs.12.00%),PCSD发生率及容积显著低于对照组,PCSD肌层厚度大于对照组[(7.01±1.87)mm vs.(4.19±1.42)mm],差异均有统计学意义(P <0.05)。两组术中出血量、肛门排气时间、住院时间、术后恶露持续时间及月经紊乱发生率相比,差异无统计学意义(P> 0.05)。结论 首次剖宫产孕妇采用双层缝合可显著降低其切口愈合不良及PSCD的发生率,对孕妇产后子宫恢复有积极作用。
Objective To observe and analyze the effect of different suturing methods on the healing of lower uterine muscularis and uterine scar diverticulum(PCSD) in the first cesarean section.Methods A retrospective collection of 300 pregnant women who underwent the first cesarean section in Chaozhou People’s Hospital from January 2020 to January 2022 were the study subjects,which were divided into two groups according to the different suture methods of the incision after cesarean section with 150 parturients in each group.Single layer suture was used in the control group,and double layer suture was used in the observation group.The operation related indicators of the two groups of parturients were compared.The duration of lochia,the incidence of menstrual disorder,the incidence of poor wound healing,and the incidence of PCSD were compared through postoperative follow-up at least 6 months.Results The operation time of observation group was longer than that of control group [(25.38±5.38)min vs.(22.31±6.03)min],the incidence of poor incision healing was significantly lower than that of control group(3.33% vs.12.00%),the incidence and volume of PCSD were significantly lower than that of control group,and the thickness of muscle layer of PCSD was greater than that of control group [(7.01±1.87)mm vs.(4.19±1.42)mm],and the differences were statistically significant(P 0.05).Conclusion Double layer suture can significantly reduce the incidence of poor incision healing and PSCD in pregnant women who give birth to cesarean section for the first time,and it has a positive effect on postpartum uterine recovery.
