摘要
目的:对比小切口大隐静脉高位结扎腔内激光闭合与传统大隐静脉高位结扎剥脱在大隐静脉曲张中的疗效。方法:回顾性选取2019年2月-2021年2月大田县总医院大隐静脉曲张患者80例,依据手术方法分为腔内激光闭合组(小切口大隐静脉高位结扎腔内激光闭合)、传统剥脱手术组(传统大隐静脉高位结扎剥脱),均为40例,统计分析两组围手术期指标、病情严重程度、临床分级、自身满意程度、疼痛情况、临床疗效、术后并发症发生情况。结果:腔内激光闭合组患者的术中出血量少于传统剥脱手术组(P<0.05),手术时间短于传统剥脱手术组(P<0.05),腹股沟处切口长度、切口总长度均短于传统剥脱手术组(P<0.05)。腔内激光闭合组患者的静脉临床严重程度量表(venous clinical severity scale,VCSS)评分、下肢慢性静脉性疾病临床-病因-解剖-病理生理分级(clinical signs-etiologic classification-anatomic distribution-pathophysiologic dysfunction classification,CEAP)均低于传统剥脱手术组(P<0.05),自身满意程度评分高于传统剥脱手术组(P<0.05)。腔内激光闭合组患者的VAS评分低于传统剥脱手术组(P<0.05)。腔内激光闭合组和传统剥脱手术组患者的痊愈率95.00%(38/40)、87.50%(35/40)比较差异无统计学意义(χ^(2)=0.626,P>0.05)。腔内激光闭合组和传统剥脱手术组患者的术后并发症发生率7.50%(3/40)、22.50%(9/40)比较差异无统计学意义(χ^(2)=3.529,P>0.05)。结论:与传统大隐静脉高位结扎剥脱相比,小切口大隐静脉高位结扎腔内激光闭合在大隐静脉曲张中的临床疗效较显著。
Objective:To compare the efficacy of small incision high saphenous vein ligation and endoluminal laser closure and traditional saphenous vein high ligation and stripping in the treatment of varicosis of great saphenous vein.Method:From February 2019 to February 2021,80 patients with varicosis of great saphenous vein in Datian County General Hospital were retrospectively selected,they were divided into endoluminal laser closure group(small incision high saphenous vein ligation and endoluminal laser closure)and traditional stripping group(traditional saphenous vein high ligation and stripping)according to surgical methods,40 cases in each group.The perioperative indicators,disease severity,clinical grade,self-satisfaction,pain,clinical efficacy and postoperative complications were statistically analyzed.Result:The intraoperative blood loss of the endoluminal laser closure group was less than that of the traditional stripping group(P<0.05),the operation time was shorter than that of the traditional stripping group(P<0.05),and the incision length at the groin and total incision length were shorter than those in the traditional stripping group(P<0.05).The venous clinical severity score(VCSS)score and clinical signs-etiologic classification-anatomic distribution-pathophysiologic dysfunction classification(CEAP)of chronic venous disease of the lower extremities grade were lower than that of the traditional stripping group(P<0.05),and the self-satisfaction score was higher than that of the traditional stripping group(P<0.05).The VAS score in the endoluminal laser closure group was lower than that in the traditional ablation surgery group(P<0.05).There was no significant difference in the recovery rates of 95.00%(38/40)and 87.50%(35/40)between the endoluminal laser closure group and the traditional ablation surgery group(χ^(2)=0.626,P>0.05).There was no significant difference in the incidence of postoperative complications of 7.50%(3/40)and 22.50%(9/40)between the endoluminal laser closure group and the traditional ablat
