摘要
目的:探讨个体化营养干预对低出生体重儿营养状况、骨代谢指标及喂养不良反应的影响。方法:收集我院2020年4月—2022年8月出生的100例低出生体重早产儿为研究对象,随机分为观察组和对照组各50例,2组早产儿胎龄、性别、分娩方式、出生体重、体长、头围间无差异,具有可比性。对照组低出生体重儿给予常规鼻饲间歇喂养,观察组低出生体重儿根据情况给予个体化营养支持。比较2组低出生体重儿干预前后血清白蛋白(ALB)、前白蛋白(PA)、视黄醇结合蛋白(RBP)、降钙素(OC)、碱性磷酸酶(ALP)和钙水平变化差异,以及不良反应发生情况的差异。结果:经营养干预后,2组低出生体重儿血清营养指标和骨代谢水平均较干预前增高(P<0.05),且观察组低出生体重儿血清ALB、PA、RBP、OC、ALP水平的增高水平高于对照组低出生体重儿,而不良反应发生率低于对照组低出生体重儿,差异均有统计学意义(P<0.05)。结论:个体化营养干预方式可改善低出生体重儿营养状况和骨代谢指标,且不良反应发生率较低,在临床实际中,可对低出生体重儿给予个体化的营养干预方式,以促进其营养吸收,改善骨代谢,促进生长发育。
[Objective]To investigate the effects of individualized nutritional interventions on nutritional status,bone metabolism indicators and adverse feeding reactions in low birth weight infants.[Method]Totally 100 low birth weight preterm infants born from April 2020 to August 2022 in our hospital were collected as study subjects and randomly divided into 50 cases each in the observation group and the control group.There were no differences in gestational age,sex,mode of delivery,birth weight,body length and head circumference between the two groups,which were comparable.The low birth weight infants in the control group were given regular nasal intermittent feeding,while the low birth weight infants in the observation group were given individualized nutritional support according to the situation.The differences in serum albumin(ALB),prealbumin(PA),retinol binding protein(RBP)levels,calcitonin(OC),alkaline phosphatase(ALP)and calcium levels before and after the intervention were compared between the two groups of low birth weight infants,as well as the differences in the occurrence of adverse effects.[Result]After nutritional intervention,the serum nutritional indexes and bone metabolism levels of low birth weight infants in both groups were higher than those before intervention(P<0.05),and the increased levels of serum ALB,PA,RBP,OC and ALP in the observation group were higher than those in the control group of low birth weight infants,while the incidence of adverse reactions was lower than that in the control group of low birth weight infants,and the differences were statistically significant(P<0.05).[Conclusion]The individualized nutritional intervention approach can improve the nutritional status and bone metabolism index of low birth weight infants,and the incidence of adverse effects is low.In clinical practice,individualized nutritional intervention approach can be given to low birth weight infants to promote their nutritional absorption,improve bone metabolism and promote growth and development.
