北京市顺义区230例0~1岁高危儿体格生长、神经心理发育情况调查分析

Study on the Correlation of Physical Growth and Neuropsychological Development of 230 High-risk Children Aged 0~1 Years Old in Shunyi District of Beijing

摘要 目的:分析0~1岁高危儿体格生长、神经心理发育相关影响因素,为高危儿的早期发育干预提供理论依据。方法:选取2021年9月—2022年6月在北京市顺义区妇幼保健院儿童早期发展中心建立高危儿管理档案并完成长期随访的230例0~1岁高危儿,在体检时进行体重、身长测量,并在12月龄时使用儿心量表评估其发育商(DQ)。结果:不同出生胎龄儿童1、3、6、9、12月龄时的身长比较,差异均有统计学意义(F值分别为74.6、42.7、35.4、18.1、7.6,P<0.05)。不同出生胎龄儿童1、3、6、9月龄时的体重比较,差异均有统计学意义(F值分别为64.2、24.8、9.8、6.8,P<0.05)。不同胎龄儿童大运动、精细动作、适应能力、语言、社交行为5个能区的DQ值比较,差异均有统计学意义(F值分别为19.9、5.1、11.5、9.2、7.7,P<0.05);除精细动作外,不同出生体重儿童大运动、适应能力、语言、社交行为4个能区的DQ值比较,差异均有统计学意义(F值分别为6.9、6.9、5.9、5.9,P<0.05),胎龄<34周的儿童大运动DQ值评分偏低,出生体重<1500 g的儿童大运动、适应能力、语言DQ值评分偏低。结论:早产儿、低出生体重的高危儿体格发育、神经系统发育应格外重视,应定期监测并进行及时干预。 OBJECTIVE To analyze the physical growth and neuropsychological development of 0~1 years old children analyzed,and the related influencing factors were analyzed to provide a theoretical basis for the early development intervention of high-risk children.METHODS 230 high-risk infants who visited the Early Childhood Development Center of Shunyi District Maternal and Child Health Hospital in Beijing from September 2021 to June 2022 were selected for weight and length measurements.Evaluate developmental quotient(DQ)using the Pediatric Heart Scale at 12 months of age.RESULTS The body length of children of different gestational ages at 1,3,6,9 and 12 months was statistically significant(F=74.6,42.7,35.4,18.1,7.6,P<0.05).The weight of children of different gestational ages at 1,3,6 and 9 months was statistically significant(F=64.2,24.8,9.8,6.8,P<0.05).There were significant differences in DQ values in 5 energy areas of children of different gestational ages(F=19.9,5.1,11.5,9.2,7.7,P<0.05).In addition to fine motor movements,the DQ scores of the other 4 functional areas were compared,and the difference was statistically significant(F=6.9,6.9,5.9,5.9,P<0.05).Children with gestational age<34 weeks had a lower DQ score on gross motor,adaptability and language,while those with birth weight<1500 g had a lower DQ score on gross motor,adaptability and language.CONCLUSION Special attention should be paid to the physical development and nervous system development of premature infants and high-risk infants with low birth weight,and regular monitoring and timely intervention should be carried out.
