摘要
目的比较保乳术与改良根治术治疗早期乳腺癌患者的临床效果。方法选取2017年6月至2019年6月在惠州市中大惠亚医院普外科接受治疗的86例早期乳腺癌患者作为研究对象,按照随机数字表法分为对照组(改良根治术)和观察组(保乳术),每组各43例。比较两组患者术后乳房美观程度、围手术期指标、生活质量和术后2年生存情况。结果观察组患者的术中出血量少于对照组,住院时间及手术时间短于对照组,差异有统计学意义(P<0.05);观察组患者的术后乳房美观优良率高于对照组,差异有统计学意义(P<0.05);观察组患者各项生命质量测定量表评分高于对照组,差异有统计学意义(P<0.05);术后2年,两组患者的复发率、转移率及生存率比较,差异无统计学意义(P>0.05),绘制Kaplan-Meier生存曲线显示,两组患者的生存时间比较,差异无统计学意义(P>0.05)。结论早期乳腺癌患者使用保乳术治疗的乳房美观程度更好,可有效缩短手术时间,促进术后恢复,且长期预后无明显差别,可根据患者情况选择手术方式。
Objective To compare the clinical effect of breast conserving surgery and modified radical mastectomy in the treatment of early breast cancer patients.Methods A total of 86 patients with early breast cancer who were treated in the Department of General Surgery,Huiya Hospital of Sun Yat-sen University,from June 2017 to June 2019 were selected as the research objects,and they were divided into the control group(modified radical mastectomy)and the observation group(breast conserving surgery)according to the random number table method,with 43 patients in each group.The postoperative breast beauty,perioperative indicators,quality of life and 2-year survival situation were compared between the two groups.Results The amount of intraoperative blood loss in the observation group was less than that in the control group,and the length of hospital stay and operation time were shorter than those in the control group,with statistically significant differences(P<0.05).The aesthetic rate of breast in the observation group was higher than that in the control group,with a statistically significant difference(P<0.05).The scores of quality of life questionnare-core 30 in the observation group were higher than those in the control group,with statistically significant differences(P<0.05).There were no statistically significant differences in the recurrence rate,metastasis rate and survival rate between the two groups two years after surgery(P>0.05).The Kaplan-meier survival curve showed that there was no significant difference in the survival time between the two groups(P>0.05).Conclusion Breast conserving surgery can effectively shorten the operation time,promote postoperative recovery,and no significant difference is observed in the long-term prognosis of patients with early breast cancer.Therefore,surgical methods can be selected according to the situation of patients.
