摘要
目的对导尿管相关尿路感染的病原菌分布以及耐药性状况加以探讨和分析。方法选取医院2018年1月至2020年12月收治的571例导尿管相关尿路感染患者,取患者中段尿进行病原菌培养,分析总结病原菌分布特点及主要病原菌耐药性状况。结果571例患者中革兰阳性菌检出率为22.60%,革兰阴性菌检出率为51.31%,真菌检出率为26.09%。屎肠球菌对万古霉素、利奈唑胺、奎奴普汀/达福普汀、替加环素耐药性较低。粪肠球菌对万古霉素、利奈唑胺、呋喃妥因、替加环素、氨苄西林、青霉素耐药性较低。金黄色葡萄球菌对万古霉素、利奈唑胺、四环素、左氧氟沙星、替加环素、红霉素耐药性较低。表皮葡萄球菌对万古霉素、利奈唑胺、呋喃妥因、四环素、替加环素、红霉素耐药性较低。大肠埃希菌对亚胺培南、呋喃妥因、哌拉西林、复方新诺明、头孢呋辛、头孢替坦、替加环素、米诺环素、美罗培南、阿米卡星耐药性较低。肺炎克雷伯菌对亚胺培南、哌拉西林/他唑巴坦、复方新诺明、头孢吡肟、头孢替坦、妥布霉素、替加环素、米诺环素、美罗培南、阿米卡星耐药性较低。铜绿假单胞菌对哌拉西林、哌拉西林/他唑巴坦、头孢他啶、头孢吡肟、妥布霉素、左氧氟沙星、庆大霉素、环丙沙星、粘菌素、阿米卡星耐药性较低。鲍曼不动杆菌对哌拉西林、哌拉西林/他唑巴坦、替加环素、粘菌素、阿米卡星耐药性较低。结论导尿管相关尿路感染的病原菌以革兰阴性菌为主,且不同病原菌耐药性存在差异,需根据实际病原菌类型及耐药性状况针对性用药。
Objective To explore and analyze the distribution and drug resistance of pathogenic bacteria in catheter-related urinary tract infection.Methods A total of 571 patients with catheter-related urinary tract infection who were admitted to the hospital from January 2018 to December 2020 were selected,and the mid-section urine of the patients was collected for pathogenic bacteria culture,and the distribution characteristics of pathogens and the drug resistance status of main pathogens were analyzed and summarized.Results Among the 571 patients,the detection rate of gram-positive bacteria was 22.60%,the detection rate of gram-negative bacteria was 51.31%,and the detection rate of fungi was 26.09%.Enterococcus faecalis was less resistant to vancomycin,linezolid,quinupristin/dalfopristin,tigecycline,and enterococcus faecalis was resistant to vancomycin,linezolid,nitrofurantoin,tigecycline,ampicillin,penicillin resistance is low.Staphylococcus aureus is low resistance to vancomycin,linezolid,tetracycline,levofloxacin,tigecycline,erythromycin,staphylococcus epidermidis is low resistance to vancomycin,linezolid the resistance to azolamide,nitrofurantoin,tetracycline,tigecycline,and erythromycin was low.The resistance of escherichia coli to imipenem,nitrofurantoin,piperacillin,co-trimoxazole,cefuroxime,cefotetan,tigecycline,minocycline,meropenem and amikacin was compared low.Klebsiella pneumoniae to imipenem,piperacillin/tazobactam,co-trimoxazole,cefepime,cefotetan,tobramycin,tigecycline,minocycline,meropenem and amikacin have low resistance.Pseudomonas aeruginosa is resistant to piperacillin,piperacillin/tazobactam,ceftazidime,cefepime,tobramycin,levofloxacin,gentamicin,ciprofloxacin,colistin,and amikacin have low resistance.Acinetobacter baumannii is resistant to piperacillin,piperacillin/tazobactam,tigecycline,colistin,amika star resistance is low.Conclusion The pathogenic bacteria of catheter-related urinary tract infection are mainly gramnegative bacteria,and the drug resistance of different pathogens is different.Targe
