摘要
目的探讨孤独症谱系障碍(ASD)患儿睡眠质量与发育及行为表现的相关性。方法收集2018年10月-2020年1月在我院就诊的ASD患儿106例(设为ASD组)和同期体检的健康儿童50名(设为对照组)的资料,采用儿童睡眠习惯问卷(CSHQ)调查2组儿童的睡眠情况,并根据CSHQ得分将ASD患儿分为睡眠正常组和睡眠不良组;统计这2组儿童的一般资料并分析睡眠不良的影响因素;采用Gesell发育量表、社交反应量表(SRS)、孤独症行为量表(ABC)及问题行为调查表对2组儿童的发育和行为表现进行评估和比较。结果ASD组睡眠不良率(60.38%)明显高于对照组(38%),且入睡潜伏期、睡眠持续时间、夜醒、异态睡眠、睡眠呼吸障碍及白天嗜睡等得分均明显高于对照组(P<0.05);年龄和居住方式是ASD患儿睡眠不良的独立影响因素;与睡眠正常组比较,睡眠不良组ASD患儿的适应性和精细运动得分明显降低,SRS得分和ABC得分明显升高,行为问题总分及情绪问题、伤人及自伤行为、重复刻板行为得分明显升高(P<0.05)。结论ASD患儿共患睡眠问题的概率高于健康儿童,不良睡眠质量与其发育和行为表现密切相关。
Objective To explore the correlation between sleep quality and developmental and behavioral performance in children with autism spectrum disorder(ASD).Methods A total of 106 children with ASD(ASD group)admitted to the Fifth Affiliated Hospital of Zhengzhou University from October 2018 to January 2020 and 50 healthy children who underwent physical examination in the same period(control group)were collected,Children's Sleep Habits Questionnaire(CSHQ)was used to investigate the sleep status of children in 2 groups,and children with ASD were divided into normal sleep group and poor sleep group according to CSHQ scores;The general data of 2 groups were collected and the influencing factors of sleep disturbance were analyzed;Gesell Developmental Scale,Social Response Scale(SRS),Autism Behavior Scale(ABC),and Problem Behavior Questionnaire were used to evaluate and compare the developmental and behavioral performance of children with ASD in 2 groups.Results The sleep disturbance rate of ASD group(60.38%)was significantly higher than that of normal group(38%),and the sleep latency,sleep duration,night wakefulness,abnormal sleep,sleep breathing disorder,and daytime sleepiness were significantly higher than those of control group(P<0.05);Age and living style were independent influencing factors of sleep disturbance in children with ASD;Compared with the normal sleep group,children with ASD in poor sleep group had significantly lower adaptability and fine motor scores,significantly higher SRS scores and ABC scores,and significantly higher total scores for behavioral problems and emotional problems,wounding and self-injury behaviors,and repetitive and stereotypical behaviors(P<0.05).Conclusion The incidence of sleep problems in children with ASD is higher than that in healthy children,and poor sleep quality is closely related to development and behavioral performance.
