婴儿发育性髋关节脱位的早期筛查与治疗效果观察 被引量:2

摘要 目的 探讨婴儿发育性髋关节脱位的早期筛查、治疗方法与效果。方法 收集2019年4月至2020年4月在唐山市妇幼保健院接受发育性髋关节脱位筛查的婴儿128例,均通过超声、X射线展开发育性髋关节脱位筛查,结果为阳性者视情况予以pavlik吊带保守治疗、闭合复位石膏固定治疗,分析筛查情况与治疗效果。结果 128例接受筛查的婴儿中,98例(76.56%)的Graf分型为I型(例), 3例(2.34%)为IIa(+)型,7例(5.47%)为IIa(-)型,4例(3.13%)为IIb型,4例(3.13%)为IIc型,3例(2.34%)为D型,5例(3.91%)为III型,4例(3.13%)为IV型,发育性髋关节脱位检出率为7.03%(9/128);9例发育性髋关节脱位患儿,7例(77.78%)接受pavlik吊带保守治疗,治疗优良率为100.00%,2例(22.22%)接受闭合复位石膏固定治疗,治疗优良率为100.00%;患儿治疗后髋臼指数均较治疗前降低,中心边缘角均较治疗前增高(P<0.05)。结论 对于婴儿发育性髋关节脱位,早期通过超声、CT实施筛查可准确诊断,并予以明确分型,依据诊断结果视情况积极展开pavlik吊带保守治疗、闭合复位石膏固定治疗可达到良好效果,促进患儿髋关节恢复。 Objective To explore the early screening, treatment methods and effects of infant developmental dislocation of the hip.Methods A total of 128 infants who were screened for developmental dislocation of the hip in our hospital from April 2019 to April 2020 were collected. All infants were screened for developmental hip dislocation with ultrasound and X-ray. Those with positive results were treated with conservative treatment of pavlik sling and closed reduction and plaster immobilization according to the situation. The screening situation and treatment effect were analyzed.Results Among the infants, 98 cases(76.56%) were Graf type I, 3 cases(2.34%) were IIa(+), 7 cases(5.47%) were IIa(-), 4 cases(3.13 %) were type IIb, 4 cases(3.13%) were type IIc, 3 cases(2.34%) were type D, 5 cases(3.91%) were type III, and 4 cases(3.13%) were type IV. The detection rate of developmental hip dislocation was 7.03 %(9/128). Of the 9 children with developmental hip dislocation, 7 cases(77.78%) received conservative treatment of pavlik sling, and the excellent and good rate was 100.00%. Another 2 cases(22.22%) received closed reduction and plaster immobilization, and the excellent and good rate was 100.00%. After treatment, the acetabular index of the children was lower than that before treatment, and the center edge angle was higher than that before treatment(P<0.05).Conclusions For infants with developmental dislocation of the hip, early screening by ultrasound and CT can be accurately diagnosed and given a clear classification. According to the diagnosis results, active conservative treatment of pavlik sling and closed reduction and plaster fixation can achieve good results and promote the hip recovery of the children.
