摘要
目的探讨Ⅰ期非小细胞肺癌(NSCLC)胸腔镜手术治疗后并发症发生的相关影响因素。方法回顾性分析198例Ⅰ期NSCLC患者临床资料,所有患者均顺利完成胸腔镜手术。术后根据患者并发症发生情况,将其分为发生组与未发生组,统计2组一般资料,包括性别、年龄、合并基础疾病、发病侧、吸烟史、术中淋巴结清扫、手术切除范围、失血量,分析术后并发症发生的危险因素。结果198例Ⅰ期NSCLC患者术后有36例发生并发症,发生率为18.18%(36/198)。发生组年龄>65岁、有吸烟史、术中有淋巴结清扫、全肺切除、失血量>150 ml患者占比均高于未发生组,差异有统计学意义(P<0.05);2组性别、合并基础疾病、发病侧等对比,差异无统计学意义(P>0.05)。Logistic回归分析:年龄>65岁、有吸烟史、术中有淋巴结清扫、全肺切除、失血量>150 ml是Ⅰ期NSCLC患者胸腔镜手术治疗后并发症发生的独立危险因素(P<0.05且OR≥1)。结论Ⅰ期NSCLC患者胸腔镜手术治疗后并发症发生率较高,年龄>65岁、有吸烟史、术中有淋巴结清扫、全肺切除、失血量>150 ml是其独立影响因素,临床需予以高度重视。
Objective To investigate the related factors of postoperative complications in patients with stageⅠnon-small cell lung cancer(NSCLC)treated by thoracoscopic surgery.Methods Clinical data of 198 patients with stageⅠNSCLC were analyzed retrospectively.All patients successfully completed thoracoscopic surgery.According to the occurrence of postoperative complications,the patients were divided into occurrence group and non occurrence group.The general data of the 2 groups were counted,including gender,age,combined basic diseases,incidence side,smoking history,intraoperative lymph node dissection,surgical resection scope and blood loss.The risk factors of postoperative complications were analyzed.Results Among 198 patients with stageⅠNSCLC,36 cases had complications,and the incidence rate was 18.18%(36/198);The proportion of age>65 years old,smoking history,intraoperative lymph node dissection,total pneumonectomy and blood loss>150 ml in the occurrence group was significantly higher than that in the non occurrence group(P<0.05);There was no significant difference between the 2 groups in gender,combined with basic diseases and incidence side(P>0.05);Logistic regression analysis showed that age>65 years old,smoking history,intraoperative lymph node dissection,total pneumonectomy and blood loss>150 ml were independent risk factors for postoperative complications in patients with stageⅠNSCLC treated by thoracoscopic surgery(P<0.05 and or≥1).Conclusion The incidence of complications after thoracoscopic surgery in patients with stageⅠNSCLC is high.Age>65 years old,smoking history,intraoperative lymph node dissection,total pneumonectomy and blood loss>150 ml are independent influencing factors,which should be paid great attention in clinical practice.
