试管法红细胞致敏反应对微柱凝集法间接抗人球蛋白试验灵敏度的影响

摘要 目的:探讨红细胞致敏反应在试管内完成对微柱凝集法抗人球蛋白试验灵敏度的影响。方法:以微柱凝集法间接抗人球蛋白试验分别检测抗D(IgG)抗体梯度浓度样本盘、两份阳性室间质评样本梯度浓度样本盘和10份临床红细胞不规则抗体阳性样本(红细胞同种抗体)梯度浓度样本盘。其中:红细胞致敏反应采用两种场景对比检测,方法一:抗原细胞与待测样本按说明书要求分别加入微柱反应,方法二:红细胞致敏反应在试管内完成。另外,观察红细胞在微柱反应腔内的沉降情况。结果:抗D(IgG)抗体递度浓度样本盘检测,两种方法在1∶32和1∶64两个稀释度凝集强度均为5,而在更高的稀释度(1∶128、1∶256及1∶512)时,采用方法二反应的红细胞凝集强度明显高于方法一。室间质评样本递度浓度样本盘和临床样本递度浓度样本盘检测阳性孔凝集强度,方法一均弱于方法二。10分钟末肉眼可见红细胞明显沉降,15分钟末沉降更明显。结论:红细胞致敏反应在试管内完成,实验易于充分混匀,还可以有效地避免抗人球蛋白抗体中和游离的待测抗体,同时增加混匀次数可使致敏反应更加充分,可提高微柱凝集法间接抗人球蛋白试验的灵敏度。 Objective:To investigate the effect of erythrocyte sensitization in vitro on the sensitivity of the anti-human globulin test by microcolumn agglutination.Methods:The microcolumn agglutination was used to detect the gradient concentration of anti-D(IgG)antibody sample plates,two positive samples for external quality assessment and 10 clinical erythrocyte irregular antibody positive samples(erythrocyte alloantibody).Among them,red blood cell sensitization reaction was compared and tested in two scenarios.Method One is that antigenic cells and samples were respectively added into microcolumns according to the instructions.Method Two is erythrocyte sensitization performed in a test tube.In addition,the sedimentation of red blood cells in the microcolumn reaction chamber was observed.Results:The agglutination intensity of the two methods at 1∶32 and 1∶64 dilutions was 5.At higher dilutions(1∶128,1∶256 and 1∶512),the agglutination intensity of the red blood cells reacted by method Two was significantly higher than that by method One.The agglutination intensity of positive wells detected in the recurrent concentration sample disc of the ventricular quality assessment sample and the clinical sample in method One was weaker than that in method Two.Besides,erythrocyte sedimentation was obvious at the end of 10 minutes and more obvious at the end of 15 minutes.Conclusion:The experiment in which erythrocyte sensitization reaction completed in the test tube is easy to fully mixed.It could effectively avoid the anti-human globulin antibody neutralization of the free antibody to be tested.Meanwhile,increasing the mixing times could make the sensitization reaction more full and improve the test sensitivity of indirect anti-human globulin by using microcolumn agglutination.
