摘要
目的测算肿瘤科各工作项目的相对价值,结合门诊和住院总工作量,计算肿瘤科门诊医生和住院医生工作量转换系数。方法通过现场调查、文献研究、专家咨询方法确定肿瘤科医生6类37个工作项目和基准项目,运用层次分析法(Analytic Hierarchy Process,AHP)综合考虑各项目的工作时间、体力消耗、脑力消耗、预期风险,确定各工作项目的相对价值。结果病例书写、辅助检查、辅助检验、治疗性操作、住院常规工作、门诊诊疗工作分别以入院病历书写、心电图、生化检查、静脉注射化疗药物、查房、副主任医师门诊为基准项目,其相对价值(权重)分别为0.0955、0.0917、0.1242、0.2169、0.2389、0.2329。结论结合2021年某月门诊、住院工作量以及收费数据,经计算获得每床日标化工作量是每门诊标化工作量的6.6倍,即每床日工作量约等于6.6个门诊工作量。
Objective Calculating the relative value of each work item in the oncology department and combining outpatient and inpatient total workload,then it can workload conversion factors for oncology outpatient and resident physicians.Methods Identifying 37 work items and benchmark items in 6 categories of oncologists through field investigation,literature research,and expert consultation,and then using the analytic hierarchy process to comprehensively consider the working time,physical exertion,mental exertion,and expected risks of each item,and determine the relative value of each work item.Results Case writing,auxiliary examination,auxiliary examination,therapeutic operation,routine inpatient work,and outpatient diagnosis and treatment work are based on admission medical record writing,electrocardiogram,biochemical examination,intravenous injection of chemotherapy drugs,ward rounds,and outpatient clinic of the deputy chief physician.The values(weights)are 0.0955,0.0917,0.1242,0.2169,0.2389 and 0.2329.Conclusion Combining the outpatient and inpatient workload and charging data for a certain month in 2021,it is calculated that the daily standardized workload per bed is 6.6 times the standardized workload per outpatient clinic,the daily workload per bed is approximately equal to the workload of 6.6 outpatient clinics.
