达芬奇机器人手术患者术中发生低体温的因素分析 被引量:1

摘要 目的 探讨达芬奇机器人手术患者术中发生低体温的因素。方法 选取行达芬奇机器人手术患者167例,收集患者一般资料[年龄、性别、身高、体质量、体质量指数(BMI)]、手术过程中的相关信息[美国麻醉医师协会(ASA)分级、术前诊断、手术类型、手术时长、麻醉时长、麻醉苏醒时长、输血量及输液量],采用自动生命体征监测系统检测患者自麻醉诱导后的鼻咽部温度(核心体温)以判断是否发生低体温(核心体温<36℃),采用单因素和多因素方法分析达芬奇机器人手术患者低体温的影响因素。结果 达芬奇机器人辅助操作手术患者术中低体温发生率为67.3%;单因素分析显示未成年人(年龄<18岁)、术中尿量、术中输液量、术中冲洗量、室温、基础体温及心率是机器人手术中发生低体温的因素(P <0.05),logistic多因素回归分析结果显示,年龄、基础体温是达芬奇机器人手术患者术中发生低体温的影响因素(P <0.05)。结论 未成年人和基础体温较低患者在达芬奇机器人手术中更易发生低体温。 Objective To investigate the influential factors of intraoperative patient hypothermia during Da Vinci robotic surgery.Methods A total of 167 patients who underwent Da Vinci robotic surgery were selected.The following information was documented,including age,gender,height,body weight,body mass index(BMI),and relevant information during the surgery such as American Society of Anesthesiologists(ASA)grade,preoperative diagnosis,types of surgery,operation duration,duration of anesthesia,wake-up time from anesthesia,blood transfusion volume,and infusion volume.Automatic vital sign monitoring system was applied to detect nasopharyngeal temperature(core temperature)of patients after the induction of anesthesia to determine whether hypothermia occurs(core temperature<36℃).Univariate and multivariate analyses were used to analyze the influential factors of intraoperative patient hypothermia during Da Vinci robotic surgery.Results The incidence rate of intraoperative hypothermia in patients undergoing Da Vinci robotic surgery was 67.3%.Univariate analysis showed that the minors(age<18 years old),intraoperative urine volume,intraoperative infusion volume,intraoperative flushing fluid volume,room temperature,basal body temperature,and heart rate were the factors for hypothermia during robotic surgery(P<0.05).Logistic regression analysis revealed that age and basal body temperature were the influential factors of hypothermia in patients undergoing robotic surgery(P<0.05).Conclusion The minors and the patients with lower basal body temperature are more likely to have hypothermia during Da Vinci robotic surgery.
