摘要
目的 探讨缬沙坦联合硝苯地平控释片治疗冠状动脉粥样硬化性心脏病(CHD)合并高血压患者的疗效及血脂、同型半胱氨酸(Hcy)的影响。方法 前瞻性选取2018年8月-2020年6月我院79例CHD合并高血压,简单随机化分组,对照组39例采取硝苯地平控释片,研究组40例采取缬沙坦+硝苯地平控释片。观察2组疗效、不良反应及治疗前后血脂指标[总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)]、血压指标[收缩压(SBP)、舒张压(DBP)]、血清Hcy、脂蛋白相关性磷脂酶A2(LP-PLA2)。结果 研究组治疗总有效率高于对照组(P<0.05);治疗后研究组LDL-C、TG、TC、SBP、DBP低于对照组(P<0.05);治疗后研究组血清Hcy、LP-PLA2水平低于对照组(P<0.05);研究组不良反应发生率与对照组比较,无统计学差异(P>0.05)。结论 缬沙坦联合硝苯地平控释片有助于下调CHD合并高血压患者血清Hcy、LP-PLA2水平,控制血脂及血压,提高治疗效果,且安全性较高。
Objective To investigate the efficacy of valsartan combined with nifedipine controlled release tablets in the treatment of patients with coronary atherosclerotic heart disease(CHD)complicated with hypertension and the influence of blood lipid and homocysteine(Hcy).Methods From August 2018 to June 2020,79 patients with hypertension and nifedipine combined with CHD in our hospital were randomly divided into two groups.The curative effect, adverse reactions, blood lipid indexes[total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL-C)],blood pressure indexes[systolic blood pressure(SBP),diastolic blood pressure(DBP)],serum Hcy and lipoprotein associated phospholipase A2(Lp-PLA2)before and after treatment were observed.Results The total effective rate of the study group was higher than that of the control group(P<0.05);After treatment, LDL-C,TG,TC,SBP and DBP of the study group were lower than those of the control group(P<0.05);After treatment, serum Hcy and Lp-PLA2 levels of the study group were lower than those of the control group(P<0.05);There was no significant difference in the incidence of adverse reactions between the study group and the control group(P>0.05).Conclusion Valsartan combined with nifedipine controlled-release tablets can help to reduce the levels of serum Hcy and Lp-PLA2 in patients with CHD and hypertension, control blood lipid and blood pressure, and improve the therapeutic effect with high safety.
