摘要
目的基于文献计量学方法研究中成药不良反应发生特点及其影响因素,探讨中成药药学监护路径建立中的关键步骤。方法检索2011年1月1日—2020年12月31日期间知网(China National Knowledge Infrastructure,CNKI)数据库中与中成药的不良反应、不合理应用相关的文献报道,利用SPSS 19.0和VOS viewer软件对文献基本信息、中成药不良反应发生特点及不合理使用情况等信息进行统计、分析。结果共计纳入111篇文献,发布单位主要为医疗机构(108篇);中成药不良反应数据中女性(54.69%)多于男性,临床表现多以用药24 h内出现(62.31%)的程度一般的过敏反应(38.97%)和消化道反应(21.19%)为主;常见不合理使用情况为用药不适宜(32.7%)及用法用量不适宜(21.79%)。结论中成药不良反应研究多集中在临床表现的描述性分析,缺乏对影响不良反应发生因素的关联性分析;现有数据提示建立中成药药学监护路径时应尽量全程化、分阶段、针对性管理。
Objective To analyze the characteristics and inducing factors of adverse reactions(ADRs)of Chinese patent medicine based on bibliometrics and explore the key steps in the establishment ofthe pharmaceutical care path of Chinese patent medicine.Methods The research reports related to ADRs and unreasonable application of Chinese patent medicines published from January 1,2011,to December 31,2020,were searched in CNKI.SPSS 19.0 and VOS Viewer were used for statistics and analysis of the basic information,the characteristics of ADRs of Chinese patent medicines,and their unreasonable application.Results A total of 111 research articles were included,which were mainly published by medical institutions(n=108).In terms of gender,females(54.69%)accounted for a higher proportion than males in all the ADRs of Chinese patent medicines.Most of the clinical manifestations were general allergic reactions(38.97%)and digestive tract reactions(21.19%)that appeared within 24 hours of medication(62.31%).The common cases of unreasonable application were inappropriate medication(32.7%)and inappropriate usage and dosage(21.79%).Conclusion The research on ADRs of Chinese patent medicines mostly focuses on the descriptive analysis of clinical manifestations,and lacks the correlation analysis of the factors affecting the occurrence of ADRs.Existing data suggest that the pharmaceutical care path for Chinese patent medicine should be managed in a whole process,in stages,and in a targeted manner.
