摘要
目的探讨大活血胶囊治疗跟骨骨折早期肿胀的临床疗效。方法选取2019年6月—2020年6月期间芜湖市中医医院收治的跟骨骨折患者100例,采用随机摸球法分为对照组和治疗组,每组各50例。两组患者均行手术治疗,术后对照组给予西医常规治疗(静脉滴注250ml 20%甘露醇,口服醋氯芬酸肠溶胶囊),在此基础上治疗组加用大活血胶囊治疗,两组患者均连续治疗7 d。评价两组患者临床疗效及药物不良反应发生率,观察两组患者首次出现皮纹征的时间及并发症发生情况,观察两组患者治疗前后患肢肿胀程度、疼痛程度、血液流变学指标(全血黏度、纤维蛋白原及红细胞压积)的变化。结果治疗后治疗组总有效率94.00%(47/50)明显高于对照组76.00%(38/50),差异有统计学意义(P<0.05)。治疗7 d后,两组患者患肢肿胀程度、VAS评分明显低于治疗前,差异有统计学意义(P<0.05);且治疗组患肢肿胀程度、VAS评分明显低于对照组,差异有统计学意义(P<0.05)。治疗组首次出现皮纹征的时间明显低于对照组,差异具有统计学意义(P<0.05)。治疗7 d后,两组患者全血黏度、纤维蛋白原及红细胞压积水平均低于治疗前,差异有统计学意义(P<0.05);且治疗组全血黏度、纤维蛋白原及红细胞压积明显低于对照组,差异有统计学意义(P<0.05)。治疗组张力性水泡(16.00%)及伤口愈合不良(6.00%)的发生率明显低于对照组张力性水泡(40.00%)及伤口愈合不良(30.00%)发生率,差异有统计学意义(P<0.05)。治疗期间,两组患者均未出现明显药物不良反应。结论大活血胶囊能明显加快跟骨骨折术后早期肿胀消退,改善患者血液流变学,减轻疼痛,且安全性可靠。
Objective To explore the clinical effect of Dahuoxue Capsules on early postoperative swelling of calcaneal fracture.Methods A total of 100 patients with calcaneal fracture who were admitted to Wuhu Traditional Chinese Medicine Hospital from June 2019 to June 2020 were randomized into control group and observation group with the touch ball method,50 in either group.Patients in both groups underwent surgical treatment.The control group was treated with conventional western medicine(intravenous infusion of 250 mL 20%mannitol,oral Aceclofenac Enteric-coated Capsules),and the observation group with Dahuoxue Capsules in addition to the conventional western medicine in the control group.The treatment lasted 7 days in both groups.The clinical efficacy and the incidence of advers ereactions were evaluated in both groups,and the time of the first occurrence of the dermoid sign and the occurrence of complications in the two groups were observed.The swelling,pain,and hemorheological indexes(whole blood viscosity,fibrinogen,and hematocrit)were observed before and after treatment in the two groups.Results The total effective rate of observation group(47/50,94.00%)washigher than that of control group(38/50,76.00%)(P<0.05).After 7 days of treatment,the swelling degree and VAS score of affected limbswerelower than those before treatment in both groups(P<0.05)and were lower in the observation groupthanin the control group(P<0.05).Thefirstdermoid signoccurred earlier in the observation groupthanin the control group(P<0.05).Aftertreatment,the whole blood viscosity,fibrinogen,and hematocritwere lower than those before treatment in both groups(P<0.05)and were lower in the observation groupthanin the control group(P<0.05).The incidence of tension blisters(16.00%)and poor wound healing(6.00%)in the observation group were significantly lower than those in the control group(40.00%,30.00%,respectively)(P<0.05).During the treatment,no obvious drug-inducedadversereactions occurred in either group.Conclusion Dahuoxue Capsules is safe,which ca
