摘要
目的观察穴位注射联合新bobath技术治疗卒中后上肢高肌张力的临床疗效。方法选取2019年1月—2021年4月期间铜陵市第二人民医院和铜陵市人民医院收治的84例脑卒中后上肢高肌张力患者,按随机数字表法分为对照组(新bobath技术治疗,42例)和研究组(穴位注射联合新bobath技术治疗,42例)。治疗1个月后,观察比较两组患者临床疗效,治疗前后改良Ashworth量表(Modified ashworth scales,MAS)分级,Fugl-Meyer运动功能(Fugl-Meyer assessment-upper extremity,FMA)、上肢动作研究量表(Action research arm test,ARAT)以及改良巴氏量表(Modified barthel Index,MBI)评分,中医症状积分(躯体感觉、肢体活动、身体本能)。结果治疗后研究组总有效率97.50%(39/40)明显高于对照组84.21%(32/38),差异有统计学意义(P<0.05)。治疗后两组患者MAS分级均优于治疗前,差异有统计学意义(P<0.05);且研究组效果明显优于对照组,差异有统计学意义(P<0.05)。治疗后两组患者FMA、ARAT、MBI评分均较治疗前升高,差异有统计学意义(P<0.05);且研究组FMA、ARAT、MBI评分均明显高于对照组,差异有统计学意义(P<0.05)。治疗后两组患者躯体感觉、肢体活动、身体本能等中医症状积分均较治疗前降低,差异有统计学意义(P<0.05);且研究组均明显低于对照组,差异有统计学意义(P<0.05)。结论穴位注射联合新bobath技术治疗可更有效的改善脑卒中后上肢高肌张力患者的肌张力状态,提高患侧肢体的运动功能,促进症状缓解,疗效更佳。
Objective To observe the clinical effect of acupoint injection combined with new Bobath technique on the treatment of post-stroke upper limb hypertonia.Methods Eighty-four patients with post-stroke upper limb hypertonia treated in Tongling Second People′s Hospital and Tongling People′s Hospital from January 2019 to April 2021were included and divided into a control group(treated with the new Bobath technique,42 cases)and a study group(treated with acupoint injection combined with the new Bobath technique,42 cases)by the random number table method.After 1 month of treatment,the clinical effects,modified ashworth scale(MAS)grades,Fugl-Meyer assessment-upper extremity(FMA)scores,action research arm test(ARAT)scores,Modified Barthel Index(MBI)scores,and traditional Chinese medicine(TCM)symptom scores(somatic sensation,physical activity,and body instinct)of the two groups before and after treatment were compared.Results The total effective rate of the study group was 97.50%(39/40),which was higher than that of the control group[84.21%(32/38)](P<0.05).After treatment,the MAS grades of both groups decreased as compared with that before treatment(P<0.05),and the effect of the study group was better than that of the control group(P<0.05).After treatment,the FMA,ARAT,and MBI scores increased in both groups as compared with those before treatment(P<0.05),and the scores of the study group were higher than those of the control group(P<0.05).After treatment,the TCM symptom scores(somatic sensation,physical activity,and body instinct)were lower in both groups(P<0.05),and the scores of the study groups were lower than those of thecontrol group(P<0.05).Conclusion Acupoint injection combined with the new Bobath technology can effectively improve the muscle tone of patients with post-stroke upper limb hypertonia,improve the motor function of the affected limb,and promote symptom relief,thereby exerting a better curative effect.
